Function and Efficacy

Nimodipine is a Ca2 channel blocker. Under normal circumstances, the contraction of smooth muscle depends on the entry of Ca2 into the cell, causing the depolarization of the transmembrane current. Nimodipine achieves the purpose of relieving vasospasm by effectively preventing Ca2 from entering the cell and inhibiting the contraction of smooth muscle. Animal experiments have shown that the effect of nimodipine on cerebral arteries is much stronger than that on arteries in other parts of the body, and because it has a high lipophilic characteristic, it is easy to pass through the blood-brain barrier. When used for the treatment of subarachnoid hemorrhage, the concentration in cerebrospinal fluid can reach 12.5ng/ml. It can be inferred that it can be used clinically to prevent vasospasm after subarachnoid hemorrhage, but the mechanism of action of this drug in human application is still unclear. In addition, it also has the effect of protecting and promoting memory and promoting intellectual recovery. Therefore, it can selectively act on cerebral vascular smooth muscle, dilate cerebral blood vessels, increase cerebral blood flow, and significantly reduce ischemic brain damage caused by vasospasm.

Indication

It is suitable for improving blood circulation in the recovery period of cerebral vasospasm after subarachnoid hemorrhage of various reasons and acute cerebrovascular disease.

Usage and Dosage

1 Ischemic cerebrovascular disease: Oral, 30-120 mg (1.5-6 tablets) per day, divided into 3 times, for one month. 2 Migraine: Oral, 40 mg (2 tablets) per time, 3 times a day, 12 weeks as a course of treatment. The effective rate is 88%, and about half of the cases can be basically cured or significantly effective. It can reduce the pain degree, frequency and duration of attacks, and prevent the appearance of premonitory symptoms for vascular, tension, cluster and mixed headaches. 3 Cerebral vasospasm caused by subarachnoid hemorrhage: Oral, 40-60 mg (2-3 tablets) per time, 3-4 times a day, 3-4 weeks as a course of treatment. For patients who need surgery, stop taking the medicine on the day of surgery and continue to take it later. 4 Sudden deafness: 40-60 mg (2-3 tablets) per day, divided into 3 times, 5 days as a course of treatment, generally 3-4 courses of treatment. 5 Mild and moderate hypertension: Patients with hypertension and the above-mentioned cerebrovascular diseases can be given priority. Oral administration: start with 40 mg (2 tablets) 3 times a day, the maximum daily dose is 240 mg (12 tablets).

Adverse Reactions

A large number of clinical practices have shown that about 11.2% of patients with subarachnoid hemorrhage experience adverse reactions when treated with nimodipine. The most common adverse reactions are: 1. Decreased blood pressure, the degree of which is related to the drug dose. 2. Hepatitis. 3. Skin tingling. 4. Gastrointestinal bleeding. 5. Thrombocytopenia. 6. Occasionally, transient dizziness, headache, facial flushing, vomiting, gastrointestinal discomfort, etc.

Precautions

Patients with severe liver damage are contraindicated. It is not recommended to take nimodipine and anti-epileptic drugs at the same time. The drug can be secreted by breast milk and should not be used by breastfeeding women.

Special Population Medication

Precautions for children: Not yet clear. Precautions for pregnancy and lactation: 1. The drug can be secreted by breast milk, so it is not suitable for lactating women. 2. Animal experiments indicate that this product is teratogenic. Precautions for the elderly: Not yet clear.

Drug Interactions

1. When used in combination with other calcium ion antagonists that act on the cardiovascular system, it can increase the effectiveness of other calcium ion antagonists. 2. When nimodipine 90 mg/day is used in combination with cimetidine 1000 mg/day for more than one week, the blood concentration of nimodipine can increase by 50%, which may be related to the inhibition of nimodipine metabolism by cimetidine in the liver cytochrome P450.
