Function and Efficacy

1 Pharmacodynamics Magnesium ions can inhibit the activity of the central nervous system, inhibit the release of acetylcholine at the motor nerve-muscle junction, block the conduction at the neuromuscular junction, reduce or relieve muscle contraction, and at the same time have a dilating effect on vascular smooth muscle, dilate spasmodic peripheral blood vessels, and lower blood pressure. Therefore, it has a preventive and therapeutic effect on eclampsia, and also has an inhibitory effect on uterine smooth muscle contraction, and can be used to treat premature delivery. 2 Pharmacokinetics It takes effect 20 minutes after intramuscular injection and takes effect almost immediately after intravenous injection. The effect lasts for 30 minutes, and the effective blood magnesium concentration for the treatment of preeclampsia and eclampsia is 2-3.5mmol/L, and the effective blood magnesium concentration for the treatment of premature delivery is 2.1-2.9mmol/L, with large individual differences. Appearance

This product is a colorless clear liquid.

Indication

It can be used as an anticonvulsant. It is often used for pregnancy-induced hypertension. It lowers blood pressure, treats preeclampsia and eclampsia, and is also used to treat premature birth.

Usage and Dosage

1. The first dose for the treatment of moderate to severe pregnancy-induced hypertension, preeclampsia and eclampsia is 2.5-4 g, diluted with 20 ml of 25% glucose injection, slowly injected intravenously within 5 minutes, and then maintained by intravenous drip of 1-2 g per hour. The total amount for 24 hours is 30 g, which is monitored based on knee tendon reflex, respiratory rate and urine output. 2. The dosage and method for the treatment of premature birth are similar to those for the treatment of pregnancy-induced hypertension. The first loading dose is 4 g; diluted with 20 ml of 25% glucose injection, slowly injected intravenously within 5 minutes, and then 60 ml of 25% magnesium sulfate injection, added to 1000 ml of 5% glucose injection, and then intravenously dripped at a rate of 2 g per hour until 2 hours after uterine contractions stop, and then oral beta-adrenergic receptor agonists are maintained.

Adverse Reactions

1. Intravenous injection of magnesium sulfate often causes symptoms such as twitching, sweating, and dry mouth. Rapid intravenous injection can cause nausea, vomiting, palpitations, dizziness, and nystagmus in some cases. Slowing down the injection speed can make the symptoms disappear. 2. In patients with renal insufficiency and large dosages, blood magnesium accumulation can occur. When the blood magnesium concentration reaches 5mmoi/L, muscle excitability may be inhibited, sensory reactions may be sluggish, knee tendon reflexes may disappear, and breathing may begin to be inhibited. When the blood magnesium concentration reaches 6mmol/L, respiratory arrest and arrhythmias may occur, as well as heart conduction block. Further increases in concentrations can cause cardiac arrest. 3. Continuous use of magnesium sulfate can cause constipation. Some patients may develop paralytic ileus, which improves after stopping the drug. 4. In very rare cases, blood calcium levels may be reduced, and hypocalcemia may reappear. 5. Drugs that are contraindicated with magnesium sulfate include polymyxin B sulfate, streptomycin sulfate, calcium gluconate, dobutamine hydrochloride, procaine hydrochloride, tetracycline, penicillin, and nafcillin (ethoxynaphthalene penicillin).
