Cefaclor Dry Suspension
Function and Efficacy
This product is a broad-spectrum semi-synthetic cephalosporin antibiotic. Its activity against penicillinase-producing Staphylococcus aureus, group A hemolytic streptococci, viridans streptococci and Staphylococcus epidermidis is the same as that of cefadroxil, and its antibacterial effect against non-enzyme-producing Staphylococcus aureus and pneumococci is 2 to 4 times stronger than that of cefadroxil. Its activity against Gram-negative bacilli, including Escherichia coli and Klebsiella pneumoniae, is stronger than that of cefadroxil, and is similar to that of cefadroxil. Its activity against Proteus mirabilis, Salmonella and Shigella is stronger than that of cefadroxil. 2.9 to 8 mg/L of this product can inhibit all Haemophilus influenzae, including strains resistant to ampicillin. Moraxella catarrhalis and Neisseria gonorrhoeae are very sensitive to this product. Indole-positive Proteus, Serratia, Acinetobacter and Pseudomonas aeruginosa are all resistant to this product. The mechanism of action of this product is to inhibit the synthesis of bacterial cell walls.

Indication
This product is mainly suitable for respiratory system, urinary system, ENT, skin and soft tissue infections caused by sensitive bacteria.
Usage and Dosage
1. Adults: 0.25g once, 3 times a day. The dose can be doubled for patients with severe infection, but the total amount per day should not exceed 4g. Or follow the doctor's advice. 2. Children: 20-40mg/kg per day according to body weight, divided into 3 times, but the total amount per day should not exceed 1g.
Adverse Reactions
1. Common gastrointestinal reactions: soft stools, diarrhea, stomach discomfort, loss of appetite, nausea, vomiting, heating, etc. 2. Serum sickness-like reactions are more common than other antibiotics, especially in children, with typical symptoms including skin reactions and joint pain. 3. Allergic reactions: rash, urticaria, eosinophilia, vulvar itching, etc. 4. Others: mild increase in serum aminotransferase, urea nitrogen and creatinine, proteinuria, tubular urine, etc.
Precautions
Patients who are allergic to this product or other cephalosporins are contraindicated.
Special Population Medication
Precautions for children: The safety of the drug for newborns has not been determined. Precautions for pregnancy and lactation: 1. Pregnant women: Multiple reproductive studies have been conducted on mice and rats, with doses as high as 12 times the human dose, and the dose studied on ferrets was three times the maximum human dose. The results showed no evidence that cefaclor impairs fertility or endangers the fetus. However, there are no sufficient strictly controlled clinical studies on pregnant women. Because animal reproductive studies cannot fully predict human responses, pregnant women should not use this product unless it is absolutely necessary. 2. Childbirth: The effect of cefaclor on childbirth is still unclear. 3. Lactating women: After a lactating woman takes 500 mg of cefaclor orally once, a small amount of cefaclor can be detected in breast milk. The average levels at 2, 3, 4 and 5 hours after taking the drug are 0.18, 0.20, 0.21 and 0.16 mg/L respectively. Trace amounts of the drug are detected in the first hour. Note for the elderly: According to literature reports, compared with healthy adult subjects under 45 years old, 40-45% of healthy elderly subjects (over 65 years old) who took 750 mg of cefaclor alone had high AUC. And 20% of patients had low renal clearance. These differences are mainly caused by age-related renal functional diseases. In clinical studies, when elderly patients take the recommended adult dose, the clinical effect and safety are similar to those of adult patients, and the dose taken by elderly patients is no different. Therefore, unless the elderly patient is weak, malnourished or has severe renal impairment, there is generally no need to adjust the dose.
Drug Interactions
Drug/experimental interactions; in tests using Benedict's and Fehling's solutions and CLINIEST tablets rather than TES-TAPE (glucose enzyme test strips, Eli Lilly and Company, USA), glucose in the urine of patients taking cefaclor may show false positive reactions. / Rare reports of increased anticoagulant effect when cefaclor is taken with oral coagulants. As with other beta-lactam antibiotics, renal excretion of cefaclor is inhibited by probenecid.
