Function and Efficacy

Risperidone is a selective monoamine antagonist with high affinity for 5HT2 receptors, D2 receptors, α1 and α2 receptors, and H1 receptors. It also has antagonistic effects on other receptors, but they are weaker. It has low to moderate affinity for 5HT1C, 5HT1D, and 5HT1A, weak affinity for D1 and haloperidol-sensitive σ receptors, and no affinity for M receptors or β1 and β2 receptors. The mechanism of risperidone in the treatment of schizophrenia is still unclear. It is believed that its therapeutic effect is the result of the combined effect of antagonism on D2 receptors and 5HT2 receptors. Antagonism on receptors other than D2 and 5HT2 may be related to other effects of risperidone.

Appearance

This product is a colorless to slightly yellow clear liquid.

Indication

It is used to treat acute and chronic schizophrenia and other various psychotic states with obvious positive symptoms (such as hallucinations, delusions, disordered thinking, hostility, suspicion) and obvious negative symptoms (such as slow reaction, emotional indifference and social indifference, few words). It can also alleviate the emotional symptoms related to schizophrenia (such as depression, guilt, anxiety). For patients who are effectively treated in the acute phase, this product can continue to exert its clinical efficacy in the maintenance phase of treatment.

Usage and Dosage

For patients who switch to this product from other antipsychotics: when starting to use it, the original antipsychotic should be gradually discontinued. If the patient was originally using a long-acting injection of an antipsychotic, this product should be used to replace the drug at the time of the next scheduled injection. Whether the anti-Parkinson's disease drugs already used need to be continued should be reassessed regularly. Adults: once or twice a day. The recommended starting dose is 1 mg twice a day, increased to 2 mg twice a day on the second day; if tolerated, it can be increased to 3 mg twice a day on the third day. Thereafter, this dose can be maintained unchanged or further adjusted based on individual circumstances. Adverse Reactions

1. Common adverse reactions associated with taking this product are: insomnia, anxiety, agitation, headache, dry mouth. 2. Less common adverse reactions are: drowsiness, fatigue, decreased concentration, constipation, indigestion, nausea, vomiting, abdominal pain, blurred vision, abnormal penis erection, erectile dysfunction, weak ejaculation, sexual apathy, urinary incontinence, rhinitis, rash and other allergic reactions. 3. It may cause extrapyramidal symptoms, such as: muscle tension, tremor, rigidity, drooling, bradykinesia, akathisia and acute dystonia. It can be eliminated by reducing the dose or giving anti-Parkinson's syndrome drugs. 4. Occasionally, symptoms of (postural) hypotension, (reflex) tachycardia or hypertension may occur.

Precautions

Patients with known hypersensitivity to the ingredients of this product are contraindicated.

Special Population Medication

Precautions for children: There is currently insufficient clinical experience for children under 15 years old. Precautions for pregnancy and lactation: It is not clear whether this product is safe for pregnant women to take it. Animal experiments have shown that risperidone has no direct toxicity to reproduction and no teratogenic effects. Nevertheless, pregnant women should not take this product unless the benefits clearly outweigh the possible risks. It is not clear whether this product is excreted through human breast milk. Animal experiments have shown that risperidone and 9-hydroxyrisperidone are excreted through animal milk. Therefore, women taking this product should not breastfeed. Precautions for the elderly: The recommended starting dose is 0.5 mg or less per day, and the dose is gradually increased to 1 to 2 mg twice a day according to individual needs. The dose adjustment interval should be no less than 1 week, and the dose increase or decrease range is twice a day, 0.5 mg each time. Before gaining more experience, the elderly should be cautious in the process of increasing the dose.

Drug Interactions

1 This product can antagonize the effects of levodopa and other dopamine agonists. 2 Carbamazepine and other liver enzyme inducers will reduce the plasma concentration of the active ingredient of this product. Once carbamazepine or other liver enzyme inducers are stopped, the dosage of this product should be re-determined and reduced if necessary. 3 Phenothiazines, tricyclic antidepressants and some beta-blockers will increase the blood concentration of this product, but will not increase the blood concentration of the antipsychotic active ingredient. 4 When taken together with other highly protein-bound drugs, there is no clinically significant mutual replacement of plasma proteins.
