glycerin
Appearance
Colorless viscous liquid, odorless, with a warm sweet taste, and can absorb moisture.
Indication
1. Treat and prevent angina pectoris. 2. Used for acute myocardial infarction. Early application (first 24 hours) can reduce mortality and complications of anterior wall infarction, and has a beneficial effect on the dilation of the long and short diameters of the left ventricle and thinning of the ventricular wall. 3. Used for congestive heart failure. 4. Can also be used to treat hypertension.
Usage and Dosage
Adults · Conventional dose · Sublingual administration 1. Tablets: For angina pectoris, 0.25-0.5 mg sublingually at a time, which can be repeated every 5 minutes until the pain is relieved. The total amount per day should not exceed 2 mg. 2. Solution: 1% solution sublingual administration, 0.05-0.1 ml at a time, 2 ml per day. · Oral administration of nitroglycerin sustained-release tablets: 2.5 mg at a time, once every 12 hours, the effect can last for 8-10 hours. · The starting dose for intravenous infusion is 5 μg/min, and it is advisable to use an infusion pump for constant-speed infusion. If the left ventricular filling pressure or pulmonary capillary entrapment pressure is normal or lower than normal (such as patients with angina pectoris without other complications), it may be fully effective or excessive. When used for controlled hypotension or treatment of heart failure, 5 μg/min can be increased every 3-5 minutes to achieve satisfactory therapeutic effects. If it is ineffective at 20 μg/min, it can be increased by 10 μg/min, and then by 20 μg/min. Once it is effective, gradually reduce the dose and extend the dosing interval. The dosage of intravenous drip should be individualized. There is no fixed suitable dose. Each patient must determine the required dose according to the required hemodynamics. Therefore, blood pressure, heart rate and other hemodynamic parameters (such as lung entrapment) must be monitored. ·Spray administration When angina pectoris occurs, use the aerosol of this drug to spray the sublingual mucosa of the oral cavity 1-2 times (equivalent to 0.5-1mg of nitroglycerin). When using, first remove the spray cap, put the cover on the spray head, turn the bottle upside down, aim the cover at the sublingual mucosa of the oral cavity, press the valve, and the liquid will be sprayed into the oral cavity in the form of mist. ·Transdermal administration The release rate of this drug patch is generally 2.5mg-10mg/24h. 1 tablet a day, stick it on the skin of the chest, and the dose can be increased as needed. ·Transmucosal administration This drug controlled release buccal tablet, 1mg once, 3-4 times a day. Place it on the canine gums of the cheek (do not place it under the tongue) and allow it to stably dissolve within 3-5 hours. If swallowed accidentally, another 1mg should be placed. If necessary, the dose can be increased to 2.5 mg 3-4 times a day. Due to the risk of inhalation, it is not recommended to use it at bedtime.
Adverse Reactions
1. Common cardiovascular symptoms include dizziness, syncope, flushing of the cheeks and neck caused by orthostatic hypotension, and tachycardia in severe cases. 2. Blood increases nitrates and denatured hemoglobin in the blood. Large doses can cause methemoglobinemia, which manifests as cyanosis. 3. Digestive system symptoms include nausea and vomiting, but rarely dry mouth. 4. Other symptoms include headache, irritability, blurred vision, tinnitus, rash, etc.
Precautions
(1) Allergy to organic nitrates. (2) Early myocardial infarction with severe hypotension and tachycardia. (3) Acute circulatory failure. (4) Severe hypotension [systolic blood pressure <12 kPa (90 mmHg)]. (5) Obstructive hypertrophic cardiomyopathy. (6) Constrictive pericarditis, cardiac tamponade. (7) Severe anemia. (8) Glaucoma. (9) Cerebral hemorrhage or head trauma. (10) Increased intracranial pressure. (11) Severe liver and kidney dysfunction. (12) Skin patches are contraindicated for patients who are allergic to adhesives (foreign data).
