Indication

This product is used in combination with heparin to prevent cardiac ischemic events in patients with unstable angina or non-Q wave myocardial infarction. It is also suitable for patients with coronary ischemic syndrome undergoing coronary angioplasty or intracoronary plaque resection to prevent cardiac ischemic complications related to sudden occlusion of the treated coronary artery.

Usage and Dosage

This product is for intravenous use only and requires sterile equipment. This product can be used in combination with heparin and infused from the same fluid circuit. It is recommended to use a graduated infusion set to infuse this product. Care must be taken to avoid prolonged load infusion. It should also be noted that the intravenous push dose and drip rate should be calculated based on the patient's weight. In the clinical study, all patients took aspirin unless they had contraindications. 1. Unstable angina or non-Q wave myocardial infarction. Tirofiban hydrochloride injection is used in combination with heparin by intravenous infusion, with an initial 30-minute drip rate of 0.4ug/kg/min. After the initial infusion is completed, continue to maintain the drip at a rate of 0.1ug/kg/min. In the test

Adverse Reactions

1. According to literature data, the most common drug-related adverse event when this product is used in combination with heparin and aspirin is bleeding (usually reported by researchers as exudation or mild bleeding). (1) Unless otherwise contraindicated, all patients received aspirin treatment. Hemoglobin decreases by more than 50 g/L, with or without bleeding at a specific site, intracranial hemorrhage, or pericardial tamponade. (2) sect; hemoglobin decreases by 30 g/L, accompanied by bleeding at a known site, spontaneous gross hematuria, hematemesis, or hemoptysis. (3) In the PRISM-PLUS study, no intracranial hemorrhage was reported in the tirofiban hydrochloride combined with heparin group or the control group (receiving heparin treatment).

Precautions

Tirofiban hydrochloride is contraindicated in patients who are allergic to any of its components. Since inhibition of platelet aggregation can increase the risk of bleeding, tirofiban hydrochloride is contraindicated in patients with active internal bleeding, a history of intracranial hemorrhage, intracranial tumors, arteriovenous malformations, and aneurysms; it is also contraindicated in patients who have previously used tirofiban hydrochloride and have developed thrombocytopenia.

Special Population Medication

Precautions for children: The safety and effectiveness of the drug for children have not been established. Precautions for pregnancy and lactation: 1. There are no appropriate and well-controlled studies on pregnant women. Tirofiban hydrochloride can only be used during pregnancy to prove that the potential benefit to the fetus outweighs the potential risk. 2. It is not known whether tirofiban hydrochloride is excreted in human breast milk. Because many drugs can be excreted in human milk and may have adverse reactions in breastfeeding infants, the decision whether to interrupt breastfeeding or drug treatment should be made based on the importance of the drug to the mother. Precautions for the elderly: In clinical studies, the effectiveness of tirofiban hydrochloride in elderly patients (≥65 years old) was similar to that in young people (

Drug Interactions

1. The interaction between tirofiban hydrochloride and aspirin and heparin has been studied. 2. When tirofiban hydrochloride is used in combination with heparin and aspirin, the incidence of bleeding is increased compared to the use of heparin and aspirin alone (see Adverse Reactions). Caution should be exercised when tirofiban hydrochloride is used in combination with other drugs that affect hemostasis (such as warfarin) (see Precautions, Prevention of Bleeding). 3. In clinical studies, no clinically significant adverse interactions were observed when tirofiban hydrochloride was used in combination with beta-blockers, calcium antagonists, nonsteroidal anti-inflammatory drugs (NSAIDS) and nitrates. 4. In a subgroup of patients (n=762) in the PRISM study (Platelet Receptor Inhibition for Treatment of Ischemic Syndromes), the plasma clearance of tirofiban in patients receiving one of the following drugs was similar to that of patients not receiving these drugs. These drugs do not have clinically significant interactions on the plasma clearance of tirofiban. These drugs were: acebutolol, acetaminophen, alprazolam, amlodipine, aspirin, atenolol, bromazepam, captopril, diazepam, digoxin, diltiazem, docusate sodium, enalapril, furosemide, glyburide, heparin, insulin, isosorbide, levothyroxine, lorazepam, lovastatin, metoclopramide, metoprolol, morphine, nifedipine, nitrates, omeprazole, oxazepam, potassium chloride, propranolol, ranitidine, simvastatin, sucralfate, and temazepam.
