Function and Efficacy
1. Inhibit the active reabsorption of urate in the renal tubules, increase the excretion of urate, reduce the concentration of urate in the blood, and thus reduce uric acid deposition. Prevent the formation of urate crystals, reduce joint damage, and promote the dissolution of urate that has already formed. This product has no anti-inflammatory or analgesic effects and is used to treat chronic gout; 2. It can competitively inhibit the secretion of weak organic acids (such as penicillin and cephalosporin) in the renal tubules, thereby increasing the blood concentration of these antibiotics and prolonging their duration of action. It can be used as an auxiliary drug for antibiotic treatment; 3. Acute toxicity test results: The oral LD50 of rats is 1600mg/kg.

This product is white tablets.
Indication
It is used for (1) hyperuricemia with chronic gouty arthritis and tophi, but it must be ① glomerular filtration rate greater than 50-60 ml/min; ② no kidney stones or history of kidney stones; ③ non-acidic urine; ④ not taking salicylic acid drugs. (2) As an auxiliary drug for antibiotic treatment, when used with antibiotics such as penicillin, ampicillin, oxacillin, o-cloxacillin, nafcillin, etc., it can inhibit the excretion of these antibiotics, increase blood drug concentration and maintain it for a long time.
Usage and Dosage
1 Hyperuricemia of chronic gout: 0.25g once for adults, twice a day, and can be increased to 0.5g once, twice a day after one week. 2 To enhance the effect of penicillins: 0.5g once for adults, 4 times a day. Children aged 2 to 14 years or weighing less than 50kg, the first dose is 0.025g/kg of body weight or 0.7g/m2 of body surface area, and then 0.01g/kg or 0.3g/m2 each time, 4 times a day.
Adverse Reactions
1 Gastrointestinal symptoms such as nausea or vomiting occur in about 5% of users. It may occasionally cause peptic ulcers; 2 It can promote the formation of kidney stones. The urine pH should be kept between 6.0 and 6.5. Drink plenty of water and take drugs that alkalinize urine to prevent kidney stones; 3 This product may have cross-allergic reactions with sulfonamides, including rash, itchy skin, and fever, but these are rare; 4 It may occasionally cause rare adverse reactions such as leukopenia, bone marrow suppression, and liver necrosis.
Precautions
1. People who are allergic to this product and sulfonamides; 2. People with renal insufficiency; 3. People with hyperuricemia accompanied by tumors, or patients using cytotoxic anticancer drugs or radiotherapy should not use this product because it can cause acute kidney disease.
Special Population Medication
Precautions for children: Children under 2 years old are prohibited. Precautions for pregnancy and lactation: This product can pass through the placenta and appear in the umbilical cord blood. Precautions for pregnant and lactating women are prohibited. Precautions for the elderly: Elderly patients should reduce the dosage due to decreased renal function.
Drug Interactions
1. Drinking alcohol, diuretics such as chlorthalidone, uric acid, furosemide, pyrazinamide and thiazide can increase serum uric acid concentration. When this product is used with these drugs, it is necessary to adjust the dosage to control hyperuricemia; 2. When used with aspirin or other salicylates, the uric acid excretion effect of this product can be inhibited; 3. When used with indomethacin, dapsone, naproxen, etc., the blood concentration of the latter increases, and the toxicity is increased; 4. When used with various penicillins and cephalosporins, the blood concentration of the latter increases, And maintain for a long time, the toxicity is increased, especially to the kidneys; 5 When used with oral hypoglycemic drugs, the latter's effect is enhanced; 6 When used with methotrexate, the latter's blood concentration may increase and the toxicity is increased; 7 When used with nitrofurantoin, the renal tubular secretion is inhibited, which reduces the anti-infection efficacy of nitrofurantoin in urine; 8 When used with rifampicin, the two drugs are taken up by the liver in competition, so the blood concentration of rifampicin can be increased and prolonged, and the toxicity is increased. In clinical practice, it is generally not recommended to use two drugs together to increase the blood concentration of rifampicin; 9 When used with sulfonamides, the latter's excretion by the kidneys is slowed down, and the blood concentration increases. The blood concentration of sulfonamides should be tested regularly when used together for a long time.
