Aspirin Dipyridamole Sustained Release Capsules
Function and Efficacy
This product is a compound preparation of dipyridamole and aspirin. The antithrombotic effect of this product is the combined antiplatelet effect of dipyridamole and aspirin. The mechanism of action of dipyridamole is as follows: (1) At therapeutic concentrations (0.5~1.9#61549;g/ml), it can dose-dependently inhibit the uptake of adenosine by platelets, epithelial cells and red blood cells, resulting in an increase in local adenosine concentration. Adenosine acts on the A2 receptor of platelets, activates adenylate cyclase, and increases the level of cyclic adenosine monophosphate (cAMP) in platelets. Through this mechanism, platelet aggregation caused by stimulation of platelet activation factors, collagen and adenosine diphosphate is inhibited. (2) Inhibits phosphodiesterase (PDE) in various tissues. At therapeutic concentrations, dipyridamole inhibits the activity of cyclic guanosine monophosphate phosphodiesterase (cGMP-PDE), but its inhibitory effect on cAMP-PDE is weak, thereby enhancing the increase in cGMP concentration caused by endothelial relaxing factor (EDRF). The mechanism of action of aspirin is: irreversible inhibition of platelet cyclooxygenase, thereby inhibiting the production of thromboxane TXA2 (a strong inducer of platelet aggregation and vasoconstriction).

Appearance
This product is a capsule, the contents of which are yellow spherical micropellets and a white or off-white film-coated tablet; the film-coated tablet appears white or off-white after removing the coating.
Indication
This product is suitable for reducing the risk of occurrence or recurrence of stroke in patients who have suffered transient ischemic attack or ischemic stroke caused by cerebral thrombosis.
Usage and Dosage
Oral administration: Adults take 1 tablet (25 mg aspirin and 200 mg dipyridamole) twice a day, once in the morning and once in the evening. It can be taken with or without food. This product should be swallowed whole and not chewed.
Adverse Reactions
1. Systemic: allergy, fever. 2. Cardiovascular: hypotension. 3. Central nervous system: coma, dizziness, paresthesia, cerebral hemorrhage, intracranial hemorrhage, subarachnoid hemorrhage. 4. Gastrointestinal: gastritis, ulcer, gastric perforation. 5. Auditory and vestibular disorders: patients with tinnitus, deafness, and high-frequency hearing loss may not feel tinnitus, so tinnitus cannot be used as a clinical indicator of salicylic acid poisoning for such patients. 6. Heart rate and arrhythmia: tachycardia, palpitations, arrhythmia, supraventricular tachycardia. 7. Hepatobiliary system abnormalities: cholelithiasis, jaundice, abnormal liver function. 8. Abnormal nutrition metabolism: high blood sugar, dry mouth. 9. Platelet, bleeding, coagulation abnormalities: hematoma, gingival bleeding. 10. Mental abnormalities: irritability. 11. Reproduction: uterine bleeding. 12. Respiration: shortness of breath, asthma, bronchospasm, hemoptysis, pulmonary edema. 13. Sensory abnormalities: loss of taste. 14. Skin and appendage abnormalities: itching, urticaria. 15. Urogenital system abnormalities: renal insufficiency and failure, hematuria. 16. Blood vessels: flushing.
Precautions
1. It is contraindicated for patients with hypotension and myocardial infarction. 2. It is contraindicated for patients with severe liver damage, hypoprothrombinemia, vitamin K deficiency and hemophilia. 3. It should be used with caution by pregnant women.
Special Population Medication
Precautions for children: The safety of this product for children has not been established. Precautions for pregnancy and lactation: Pregnant women should use with caution. Precautions for the elderly: According to foreign literature reports, after healthy elderly people (>65 years old) use this compound preparation, the plasma concentration of dipyridamole (measured by AUC) is about 40% higher than that of people under 55 years old. It is suggested that the dosage should be appropriately adjusted when the elderly use this product.
Drug Interactions
Adenosine: It has been reported in the literature that dipyridamole can increase the blood concentration of adenosine and its cardiovascular effects. The dose of adenosine should be adjusted when necessary. Angiotensin-converting enzyme (ACE) inhibitors: Aspirin acts indirectly on the renin-angiotensin conversion pathway. When co-administered with aspirin, the sodium-lowering and blood pressure-lowering effects of ACE inhibitors are reduced. Acetazolamide: When aspirin and acetazolamide are co-administered, the serum concentration of acetazolamide may increase due to their competitive excretion in the renal tubules. Anticoagulant therapy (heparin and warfarin): Patients treated with anticoagulants have an increased risk of bleeding due to drug interactions and effects on platelets. Aspirin can displace warfarin bound to proteins, resulting in prolonged coagulation time and bleeding time. Aspirin increases the anticoagulant activity of heparin, increasing the risk of bleeding. Anticonvulsants: Salicylic acid can displace protein-bound phenytoin and valproic acid, resulting in a decrease in the total concentration of phenytoin and an increase in serum valproic acid levels. β-blockers: When taken with aspirin, they inhibit renal prostaglandins, resulting in decreased renal blood flow and reduced antihypertensive effects of β-blockers. Acetylcholinesterase inhibitors: Dipyridamole can counteract the anti-acetylcholinesterase effect of acetylcholinesterase inhibitors, thus potentially exacerbating myasthenia. Methotrexate: Salicylic acid can inhibit the renal clearance of methotrexate and cause bone marrow toxicity, especially in the elderly or those with renal insufficiency. Diuretics: When aspirin and diuretics are used together in patients with renal and cardiovascular disease, the efficacy of diuretics may be reduced. Nonsteroidal anti-inflammatory drugs (NSAIDs): When aspirin is used together with other nonsteroidal anti-inflammatory drugs, it may increase bleeding or lead to decreased renal function. Oral hypoglycemic drugs: Moderate doses of aspirin can increase the effectiveness of hypoglycemic drugs and lead to hypoglycemia. Antigout drugs (probenecid and sulfinpyrazone): Salicylic acid can eliminate the uricosuric effect of antigout drugs.
