Penicillin Sodium

Function and Efficacy

Penicillin has good antibacterial effect on Streptococcus such as hemolytic Streptococcus, Streptococcus pneumoniae and Staphylococcus that does not produce penicillinase. It has moderate antibacterial effect on enterococci, and Neisseria gonorrhoeae, Neisseria meningitidis, Corynebacterium diphtheriae, Bacillus anthracis, Actinomyces bovis, Streptobacillus candida, Listeria monocytogenes, Leptospira and Treponema pallidum are sensitive to this product. This product also has certain antibacterial activity against Haemophilus influenzae and Bordetella pertussis, and other Gram-negative aerobic or facultative anaerobic bacteria are less sensitive to this product. This product has good antibacterial effect on Clostridium, Peptostreptococcus anaerobic bacteria and melanin-producing Bacteroides, but has poor antibacterial effect on Bacteroides fragilis. Penicillin exerts its bactericidal effect by inhibiting bacterial cell wall synthesis.

Indication

This product is suitable for mild and moderate infections caused by penicillin-sensitive strains.

Adverse Reactions

1. Allergic reaction: Allergic reaction to penicillin is common, including urticaria and other types of rashes, leukopenia, interstitial nephritis, asthma attacks, and serum sickness reactions; anaphylactic shock is rare. Once it occurs, it must be rescued on the spot, and the airway must be kept open, oxygen inhalation, and treatment measures such as epinephrine and glucocorticoids must be used. 2. Toxic reaction: It is rare, but when a large dose of this product is intravenously infused or intrathecally administered, convulsions, muscle spasms, coma, and severe mental symptoms (penicillin encephalopathy) may occur due to excessive concentration of drugs in the cerebrospinal fluid. This reaction is more common in infants, the elderly, and patients with renal insufficiency. 3. Herxheimer's reaction and treatment contradiction: When penicillin is used to treat diseases such as syphilis and leptospirosis, symptoms may be aggravated due to the death of pathogens, which is called Herxheimer's reaction; treatment contradictions are also seen in syphilis patients, which is caused by the rapid disappearance of syphilis lesions after treatment, while tissue repair is relatively slow or the fibrous tissue in the lesion site contracts, hindering organ function. 4. Dual infection: Dual infection with penicillin-resistant Staphylococcus aureus, Gram-negative bacilli or Candida albicans may occur. 5. The use of large doses of penicillin sodium may lead to heart failure due to the intake of large amounts of sodium salt.

Precautions

It is contraindicated in patients with a history of allergy to penicillins or a positive penicillin skin test.

Drug Interactions

1 Chloramphenicol, erythromycin, tetracyclines, and sulfonamides can interfere with the activity of this product, so this product should not be used in combination with these drugs. 2 Probenecid, aspirin, indomethacin, phenylbutazone, and sulfonamides reduce the renal tubular secretion of penicillin and prolong the serum half-life of this product. Penicillin can enhance the anticoagulant effect of warfarin. 3 This product is incompatible with heavy metals, especially copper, zinc, and mercury. 4 Adding cephalothin, lincomycin, tetracycline, vancomycin, erythromycin ethylsuccinate, amphotericin B, norepinephrine, metaraminol, phenytoin sodium, hydroxyzine hydrochloride, prochlorperazine, promethazine, vitamin B group, vitamin C group, etc. to penicillin intravenous infusion will cause turbidity. 5 This product and aminoglycoside antibiotics drip in the same bottle may lead to a decrease in the antibacterial activity of both, so they cannot be placed in the same container for administration.
