Function and Efficacy

This product has a broad-spectrum antibacterial effect and has good antibacterial effect on the following bacteria: most bacteria of the Enterobacteriaceae family, including Escherichia coli, Klebsiella, Proteus, Shigella, Typhi and Salmonella, as well as Haemophilus influenzae, Neisseria, etc. It also has a certain antibacterial effect on Pseudomonas aeruginosa and Staphylococcus aureus, but only has a slight effect on Pneumococcus, various groups of Streptococci and Enterococci. In addition, it also has antibacterial activity against Mycobacterium leprae. Pefloxacin mesylate is a bactericide that acts on the A subunit of bacterial DNA helicase, inhibits DNA synthesis and replication, and causes bacterial death.

Indication

It has a strong and broad antibacterial effect and is widely used in clinical practice for various infections. Urinary tract infections: 1. Acute and chronic urinary tract infections: acute pyelonephritis, cystitis, prostatitis, complicated urinary tract infections, or bacterial infections caused by surgery. 2. Nephrotic syndrome: Foreign literature has reported that idiopathic nephropathy and syndrome were relieved after treatment with pefloxacin, so it is recommended to use this drug alone as the first-line treatment for nephrotic syndrome. Respiratory tract infections: 1. Upper respiratory tract infections 2. Complicated respiratory tract infections: acute pneumonia, acute and chronic bronchitis, lung infections caused by various reasons, and intractable bronchial asthma. Digestive system infections: 1. Usage and Dosage

Oral administration: 0.2-0.4 g per day for adults, twice a day

Adverse Reactions

Gastrointestinal reactions are common and may manifest as abdominal discomfort or pain, diarrhea, nausea or vomiting. Central nervous system reactions may include dizziness, headache, drowsiness or insomnia. Allergic reactions: rash, itchy skin, occasionally exudative erythema multiforme and angioedema, and a few patients may have photosensitivity reactions. Occasionally, the following may occur: (1) epileptic seizures, mental abnormalities, irritability, confusion, hallucinations, tremors, (2) interstitial nephritis symptoms such as hematuria, fever, and rash, (3) crystalluria, which is more common when high doses are used, and (4) joint pain. A small number of patients may experience elevated serum aminotransferase, increased blood urea nitrogen, and decreased peripheral white blood cells, which are mostly mild and transient.

Precautions

Patients who are allergic to this product and fluoroquinolones are contraindicated.

Special Population Medication

Precautions for children: The safety of this product in infants and adolescents under 18 years of age has not been determined, so it is contraindicated for patients under 18 years of age. Precautions for pregnancy and lactation: Animal experiments have not confirmed that quinolones are teratogenic, but studies on the use of drugs in pregnant women have not yet reached a clear conclusion. Since this drug can cause joint lesions in minors, it is contraindicated for pregnant women and lactating women. If this product must be used, breastfeeding should be suspended. Precautions for the elderly: It is not yet clear.

Drug Interactions

Urine alkalinizing agents can reduce the solubility of this product in urine, leading to crystalluria and nephrotoxicity. When this product is used in combination with theophylline, competitive inhibition of the binding site of cytochrome P450 may lead to a significant reduction in the elimination of theophylline in the liver, a prolonged elimination half-life (t1/2β), an increase in blood concentration, and the occurrence of symptoms of theophylline poisoning, such as nausea, vomiting, tremor, restlessness, agitation, convulsions, palpitations, etc., and should be avoided. If it cannot be avoided, theophylline blood concentration should be measured and the dose adjusted. When cyclosporine is used in combination with this product, its blood concentration increases, and the blood concentration of cyclosporine must be monitored and the dose adjusted. When this product is used in combination with the anticoagulant warfarin, it can enhance the latter's anticoagulant effect. When used in combination, the patient's prothrombin time should be closely monitored and the dose adjusted. Probenecid can reduce the secretion of this product from the renal tubules by about 50%. When used in combination, it may cause toxicity due to the increased blood concentration of this product. This product interferes with the metabolism of caffeine, resulting in reduced caffeine elimination, prolonged blood elimination half-life (t1/2β), and possible central nervous system toxicity. When used together, the patient's blood caffeine concentration should be closely monitored and the dosage adjusted. Antacids containing aluminum and magnesium can reduce the oral absorption of this product and should not be used together.
