Succinylcholine Chloride Injection
Function and Efficacy
After binding to nicotinic receptors, this product produces a stable depolarization effect, causing skeletal muscle relaxation. This product can be quickly hydrolyzed by pseudocholinesterase in the blood after entering the body, and its intermediate metabolite succinyl monocholine has a very weak muscle relaxant effect. After intravenous injection, this product first causes a short period of muscle fasciculation, starting from small muscles such as the brow and upper eyelid, to the scapula and pectoralis major, and to the upper and lower limbs. The muscle relaxant effect takes effect in 60 to 90 seconds and lasts for about ten minutes. Repeated intravenous injection or continuous drip can prolong the effect. The muscle relaxant effect takes effect in 60 to 90 seconds and lasts for about 10 minutes.

Appearance
This product is a colorless or almost colorless clear viscous liquid.
Indication
Depolarizing skeletal muscle relaxant. It can be used for endotracheal intubation during general anesthesia and to maintain muscle relaxation during surgery.
Usage and Dosage
This product must be used under conditions of assisted or controlled breathing: 1. During endotracheal intubation, 1-1.5 mg/kg, up to 2 mg/kg; 1-2 mg/kg for children, diluted with 0.9% sodium chloride injection to 10 mg per ml, intravenous or deep intramuscular injection, intramuscular injection should not exceed 150 mg at a time. 2. To maintain muscle relaxation: 150-300 mg at a time dissolved in 500 ml of 5%-10% glucose injection or 1% procaine hydrochloride injection mixed solution and intravenously dripped.
Adverse Reactions
1. Hyperkalemia This product causes muscle fiber depolarization, causing K in the cell to flow rapidly to the extracellular space. Normal human blood potassium rises by 0.2-0.5mmol/L; severe burns, soft tissue injuries, intra-abdominal infections, tetanus, paraplegia and hemiplegia, etc., under the action of this product, cause abnormal large amounts of K outflow, leading to hyperkalemia, severe ventricular arrhythmias and even cardiac arrest. 2. Cardiac effects The acetylcholine-mimetic effect of this product can cause bradycardia, nodal arrhythmias and cardiac arrest, especially when repeated large doses are administered. 3. Increased intraocular pressure This product causes spasmodic contraction of the extraocular muscles, resulting in increased intraocular pressure. 4. The highest increase in gastric pressure can reach 40cmH2O, and can cause reflux and aspiration of gastric contents in patients with full stomachs. 5. Malignant hyperthermia is more common in patients who use this product in combination with halothane; it also occurs more often in children. 6. Postoperative myalgia: Patients who rest in bed after medication have mild and less myalgia, while those who get up and move around within 1 to 2 days have severe and more myalgia. 7. It may cause increased muscle tension, most obvious in the pectoralis major, followed by the abdominal muscles, and in severe cases, it may affect the biceps and quadriceps. At this time, not only does the body's total oxygen consumption increase, it is enough to cause an increase in intragastric pressure and even intracranial pressure.
Precautions
1) It is contraindicated for patients with cerebral hemorrhage, glaucoma, retinal detachment, cataract extraction, low plasma cholinesterase, severe trauma, large-area burns, upper motor neuron damage, and hyperkalemia. (2) It should be used with caution in patients taking anticholinesterase drugs.
Drug Interactions
1) This product decomposes in alkaline solution, so it should not be mixed with sodium thiopental for injection. (2) The following drugs can reduce the activity of pseudocholinesterase and enhance the effect of this product. ① Anticholinesterase drugs. ② Anti-tumor drugs such as cyclophosphamide, nitrogen mustard, and thiotepa. ③ Local anesthetics such as procaine. ④ Monoamine oxidase inhibitors, estrogen, etc. (3) It should also be used with caution with the following drugs, such as: phenothiazines, procainamide, quinidine, kanamycin, polymyxin B, neomycin, etc. have depolarizing muscle relaxant effects and can enhance the effect of this product.
