Metoprolol Tartrate Injection

Function and Efficacy

This drug belongs to Class 2A, i.e., beta 1-receptor blockers without partial agonist activity (cardioselective beta 1-receptor blockers). It has a selective blocking effect on beta 1-receptors, no PAA (partial agonist activity), and no membrane stabilizing effect. Its effect on blocking beta 1-receptors is approximately equal to that of propranolol (PP), and its selectivity for beta 1-receptors is slightly inferior to that of atenolol. Metoprolol's effects on the heart, such as slowing heart rate, inhibiting cardiac contractility, reducing automaticity, and delaying atrioventricular conduction time, are similar to those of propranolol and atenolol (AT). Its effect on reducing elevated blood pressure and heart rate during exercise testing is also similar to that of PP and AT. Its contraction effect on vascular and bronchial smooth muscle is weaker than that of PP, so its effect on the respiratory tract is also smaller, but still stronger than that of AT. Metoprolol can also reduce plasma renin activity.

Appearance

Indication

Supraventricular tachyarrhythmias. Prevention and treatment of myocardial ischemia, tachyarrhythmias and chest pain in patients with confirmed or suspected acute myocardial infarction.

Usage and Dosage

This product is prone to sudden changes in heart rate, blood pressure and cardiac output, so it should be used with caution under ECG monitoring. 1. Emergency treatment of rapid arrhythmia: Metoprolol adult dose 5 mg, diluted with glucose solution, slowly injected intravenously, if necessary, repeated injections can be made every 10 minutes, depending on the severity of the disease, total dose 10 mg (4 to 6 hours after intravenous injection, arrhythmia has been controlled, oral capsules are used 2 to 3 times a day, each dose does not exceed 50 mg. 2. Treatment of arrhythmia during induction of anesthesia or anesthesia: slow intravenous injection, 2 mg for adults, 2 mg can be repeatedly injected, and the maximum total amount is 10 mg if necessary.

Adverse Reactions

This product can cause dizziness, vertigo, insomnia, dreaminess, depression, and dullness through the nervous system. Some cases may cause hypotension, bradycardia, dizziness, and discomfort. Rare symptoms include diarrhea, nausea, asthma, rash, and thrombocytopenia.

Precautions

Ⅱdeg; and Ⅲdeg; are contraindicated for patients with atrioventricular block, cardiogenic shock, severe bradycardia (heart rate less than 60 beats/min), systolic blood pressure less than 12kpa, heart failure, sick sinus syndrome and pregnant women.

Drug Interactions

1. Patients with catecholamine depletion (such as reserpine) may experience dizziness or postural hypotension when used in combination with this product. 2. Nifedipine and diltiazem inhibit myocardial contraction, relax smooth muscle, and have different degrees of blood pressure lowering effects. They should not be used in combination with beta-receptor blockers. 3. When used in combination with clonidine and the drug must be discontinued, this product must be discontinued first, and then clonidine should be gradually discontinued after a few days to avoid blood pressure fluctuations. 4. When used in combination with digitalis glycosides, atrioventricular block may occur and cause a slow heart rate, so close observation is required. 5. When used in combination with adrenaline, phenylephrine or sympathomimetic amines, it may cause significant hypertension, slow heart rate, and atrioventricular block, so close observation is required. 6. It can enhance the efficacy of non-depolarizing muscle relaxants such as tubocurarine chloride and galactodonium iodide, and the duration of effect is also prolonged. 7. It can affect blood sugar levels, so when used in combination with hypoglycemic drugs, the latter's dose must be adjusted. 8 When used with isoproterenol or xanthine, the efficacy of the latter two may be weakened. 9 When used with monoamine oxidase inhibitors, it may cause extreme hypotension and is contraindicated. 10 When used with phenothiazines, the blood concentrations of both drugs may increase. 11 When used with reserpine, the effects of the two drugs are additive, the beta-receptor blocking effect is enhanced, and bradycardia and hypotension may occur.
