Isosorbide Mononitrate Tablets

Isosorbide mononitrate (ISMN) is the main biologically active metabolite of isosorbide dinitrate. Like other organic nitrates, its main pharmacological action is to relax vascular smooth muscle. ISMN releases nitric oxide (NO), which is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris.

The main chemical name of this product is: 1,4:3,6-dianhydro-D-sorbitol-5-mononitrate.

This product is white tablets.

Long-term treatment of coronary heart disease; prevention of angina pectoris; treatment of persistent angina pectoris after myocardial infarction; combined with digitalis or diuretics to treat chronic congestive heart failure.

Tablets: 10-20 mg orally, 2 to 3 times a day. For severe cases, 40 mg can be used, 2 to 3 times a day.

Common adverse reactions are headaches (10% of patients), which usually disappear after continued use. Hypotension and/or dizziness (1-10%) may occur at the beginning of treatment or when the dose is increased, accompanied by dizziness, drowsiness, emission tachycardia and fatigue. Occasionally (1%), nausea, vomiting, facial flushing, skin allergies (such as rashes) may occur, which may sometimes be severe. Exfoliative dermatitis is rare. Severe hypotensive reactions to organic nitrates have been reported, including nausea, vomiting, restlessness, pallor and sweating. Collapse (sometimes accompanied by arrhythmias and syncope) is rare. Severe hypotension exacerbating angina pectoris is also rare. During treatment with Pandego (isosorbide mononitrate tablets), transient hypoxemia may occur due to redistribution of blood into underventilated alveolar areas. The phenomenon of stomach pain may be due to the fact that nitrates can relax the sphincter.

Acute circulatory failure (shock, circulatory collapse); severe hypotension (systolic blood pressure 90 mmHg); acute myocardial infarction with low filling pressure (unless under continuous hemodynamic monitoring); hypertrophic obstructive cardiomyopathy; constrictive pericarditis or cardiac tamponade; severe anemia; glaucoma; increased intracranial pressure; allergy to nitro compounds.

Precautions for children: All studies on this type of drug were conducted in adults. There is no data comparing the use of drugs in children and adults, so it is not recommended for use in children. Precautions for pregnancy and lactation: No toxic effects on embryos were observed in animal experiments, and it is not clear whether ISMN is excreted through breast milk. However, due to the lack of experience in the use of drugs in pregnant and lactating women, it should be used with caution. Precautions for the elderly: Elderly patients may be more sensitive to this type of drug and are more likely to experience reactions such as dizziness.

Combination with other vasodilators, calcium antagonists, beta-blockers, antihypertensive drugs, tricyclic antidepressants and alcohol can enhance the blood pressure lowering effect of this class of drugs.
