Description	Cinacalcet Hydrochloride is a type II calcimimetic agent which controls calcium levels in cells by allosteric activation of CaSR. In the presence of calcium ions, it can inhibit parathyroid hormone secretion by activating CaSR in parathyroid glands. Studies show that Cinacalcet Hydrochloride affects cell proliferation in CHO cells transfected with CaSRs. In addition, it is known that Cinacalcet Hydrochloride inhibits the secretion of the parathyroid hormone (PTH). Cinacalcet has been used clinically to treat secondary hyperparathyroidism due to end-stage renal disease or hypercalcemia in patients with parathyroid carcinoma.

1. General Information

Cinacalcet Hydrochloride is a pharmaceutical compound. It is a calcimimetic agent. The chemical formula of Cinacalcet Hydrochloride is C₂₂H₂₂F₃N·HCl.

2. Mechanism of Action

Cinacalcet works by increasing the sensitivity of the calcium - sensing receptor (CaSR) on the parathyroid chief cells. The parathyroid gland plays a crucial role in calcium homeostasis. When the CaSR is activated, it leads to a decrease in parathyroid hormone (PTH) secretion. In patients with conditions such as secondary hyperparathyroidism (SHPT), the parathyroid gland over - secretes PTH. By modulating the CaSR, Cinacalcet helps to reduce the elevated levels of PTH.

3. Indications

Secondary Hyperparathyroidism: It is mainly used for the treatment of secondary hyperparathyroidism in patients with chronic kidney disease (CKD) on dialysis. In these patients, abnormal calcium and phosphorus metabolism can lead to over - activation of the parathyroid gland. Cinacalcet helps to control PTH levels, which in turn can help manage bone and mineral disorders associated with CKD.

Hypercalcemia in Parathyroid Carcinoma: It can also be used to treat hypercalcemia (elevated blood calcium levels) associated with parathyroid carcinoma. In such cases, it helps to lower the calcium levels by reducing PTH secretion.

4. Pharmacokinetics

Absorption: Cinacalcet Hydrochloride is well - absorbed after oral administration. The peak plasma concentration is usually reached within 2 - 6 hours after a single dose.

Distribution: It is widely distributed in the body, with a relatively high volume of distribution. It is about 1000 L. It binds to plasma proteins to some extent, approximately 93% - 97%.

Metabolism: It is metabolized mainly in the liver through the cytochrome P450 system, specifically by CYP3A4. The metabolites are then excreted.

Excretion: The drug and its metabolites are excreted predominantly in the feces (about 80%) and to a lesser extent in the urine (about 15%).

5. Dosage and Administration

The dosage of Cinacalcet Hydrochloride depends on the indication and the patient's response. For secondary hyperparathyroidism in dialysis patients, the initial dose is usually 30 mg once daily. The dose can be adjusted based on the patient's PTH, calcium, and phosphorus levels. It is typically taken orally with or without food.

6. Side Effects

Gastrointestinal Effects: Common side effects include nausea, vomiting, and diarrhea. These symptoms may occur due to the drug's effects on the gastrointestinal tract or related to changes in calcium and PTH levels.

Hypocalcemia: There is a risk of developing hypocalcemia (low blood calcium levels). This is because the drug reduces PTH, which can lead to decreased calcium mobilization from the bones and other tissues. Regular monitoring of calcium levels is essential during treatment.
