Gamithromycin is a semi-synthetic macrolide antibiotic belonging to the azalide subclass, used primarily in veterinary medicine for the treatment and prevention of respiratory diseases in cattle and pigs.

Mechanism of Action

Gamithromycin exerts its antibacterial effects by binding to the 50S subunit of bacterial ribosomes, inhibiting protein synthesis through prevention of peptide chain elongation. This disruption leads to bacteriostatic and bactericidal activity against susceptible bacteria. Its unique chemical structure, including an alkylated nitrogen at the 7a-position of the lactone ring, facilitates rapid absorption at physiological pH and prolonged persistence in target tissues such as the lung.

Applications

Cattle: Treatment and metaphylaxis of bovine respiratory disease (BRD) caused by Mannheimia haemolytica, Pasteurella multocida, and Histophilus somni.
Pigs: Treatment of swine respiratory disease (SRD) associated with Actinobacillus pleuropneumoniae, Pasteurella multocida, and Haemophilus parasuis.
Also effective against Fusobacterium necrophorum and Dichelobacter nodosus.
Used both therapeutically and metaphylactically to control outbreaks in herds.

Safety and Side Effects

Common adverse reactions include transient injection site swelling and occasional mild pain, typically resolving within days to weeks depending on species (cattle: 3–14 days, pigs: 2 days, sheep: 2–4 days).
Hypersensitivity reactions in people with macrolide allergies should be avoided.
Not recommended for simultaneous use with other macrolides or lincosamides.
Environmental precautions advised due to toxicity to aquatic life.

Routes of Administration

Cattle and Sheep: Subcutaneous injection, typically in the neck region. For large animals, doses exceeding 10 ml should be divided among injection sites.
Pigs: Intramuscular injection, with injection volumes not exceeding 5 ml per site.
Single dose administration of 6 mg/kg body weight is standard.

Absorption: Rapid absorption with peak plasma concentrations achieved within minutes (5–15 min in pigs).
Bioavailability: High oral bioavailability (>92–98%).
Distribution: Extensive distribution with a large volume of distribution (~25 L/kg), especially accumulating in lung tissue where concentrations exceed plasma levels by over 264-fold, persisting for at least 10 days.
Protein Binding: Moderate plasma protein binding (~27%).
Metabolism and Elimination: Primarily excreted unchanged via bile; minor metabolism.
Half-life: Long plasma half-life (~4 days in pigs), supporting single-dose efficacy.
