Cisapride is a prototype serotonergic agent, evolved from a body of research that defined the key roles of serotonergic receptors in gastrointestinal motor and sensory function. Mechanism of Action

Cisapride acts as a selective 5-HT4 receptor agonist, with an IC50 value of 0.483 μM. It promotes the release of acetylcholine from the myenteric plexus, resulting in increased gastrointestinal motility. Unlike other prokinetic agents, it does not directly stimulate muscarinic or nicotinic receptors, nor does it inhibit acetylcholinesterase.

Medical Uses

Cisapride is primarily used for:

Gastroesophageal reflux disease (GERD), including the treatment and maintenance of esophagitis.

Functional dyspepsia with symptoms such as early satiety, bloating, and nausea.

Chronic constipation.

Pseudo-obstruction of the gastrointestinal tract.

Dosage and Administration

The usual adult dose is 15-40 mg per day, divided into 2-4 doses.

For severe conditions like gastroparesis or refractory constipation, higher doses may be required.

Side Effects

Common side effects include abdominal cramps, diarrhea, and headache. Rare but serious side effects may involve cardiac arrhythmias due to its potential to block hERG channels.

Safety and Withdrawal

Due to concerns about cardiac toxicity, cisapride has been withdrawn from the market in many countries, including the United States and Canada. Its use is now limited to specific cases where the benefits outweigh the risks
