Metronidazole is a nitroimidazole-class antibiotic and antiprotozoal agent widely used to treat infections caused by anaerobic bacteria and certain protozoa. It is effective against a variety of anaerobic infections, including those caused by Bacteroides, Clostridium, Fusobacterium, Peptococcus, and Peptostreptococcus species, as well as protozoal infections such as trichomoniasis, giardiasis, and amebiasis.

Mechanism of Action

Metronidazole is a prodrug that enters susceptible anaerobic microorganisms by passive diffusion. Inside these organisms, it undergoes reduction of its nitro group by intracellular electron transport proteins such as ferredoxin or flavodoxin, producing reactive nitroso free radicals. These radicals cause DNA strand breakage and inhibit nucleic acid synthesis, leading to microbial cell death. This activation process requires a low-oxygen environment, which explains metronidazole’s selectivity for anaerobic bacteria and protozoa. The exact molecular details remain partially unclear, but the drug’s cytotoxic effects are primarily due to DNA damage caused by these reactive intermediates.

Metronidazole is used to treat:
Anaerobic bacterial infections of the skin, abdomen, central nervous system, respiratory tract, reproductive organs, and bloodstream (septicemia).
Protozoal infections such as trichomoniasis, giardiasis, and amebiasis.
Bacterial vaginosis and pelvic inflammatory disease (often in combination with other antibiotics).
Clostridioides difficile colitis (mild to moderate cases when other treatments are unavailable).
Topical treatment of inflammatory lesions of rosacea.
Eradication of Helicobacter pylori in combination therapy.

Safety and Side Effects

Common side effects include nausea, metallic taste, loss of appetite, and headaches. Rare but serious adverse effects can include seizures, encephalopathy, aseptic meningitis, and optic neuropathy. Allergic reactions may also occur. Metronidazole is generally considered safe during breastfeeding, but caution is advised during the first trimester of pregnancy. It has a relatively good safety profile when used at recommended doses.

Metabolism: Approximately 60% is metabolized in the liver by oxidation to hydroxymetronidazole and a carboxylic acid derivative, both of which retain some antimicrobial activity.
Elimination: Mainly excreted via the kidneys (about 77%) and to a lesser extent in feces (about 14%). The elimination half-life in healthy adults is about 8 hours; it is longer in neonates and premature infants.
Steady-state concentrations: Achieved with repeated dosing; plasma levels are dose-proportional.
