By inhibiting PBPs, amoxicillin prevents proper cell wall formation, leading to bacterial cell lysis and death due to osmotic instability. This mechanism is specific to bacteria and does not affect mammalian cells.

Pharmacokinetics

Absorption: Oral bioavailability is approximately 60-95%, with peak plasma concentrations reached within 1 to 1.5 hours after dosing.
Distribution: Amoxicillin distributes widely into body tissues and fluids, including respiratory tract, urinary tract, skin, middle ear, sinuses, and prostate. It crosses the placenta and is excreted in breast milk in small amounts.
Protein Binding: Approximately 17% bound to serum proteins.
Metabolism: Partially metabolized in the liver to several metabolites including hydroxylated and glucuronidated forms.
Elimination: Primarily excreted unchanged in the urine via glomerular filtration and tubular secretion; about 60-78% of the dose is eliminated in urine within 6-8 hours.
Half-life: Approximately 1 to 1.5 hours in adults; longer in neonates.
Clearance: Mean clearance is about 21.3 L/h.
