What is Lubiprostone?
Lubiprostone is a prescription medication classified as a chloride channel activator used to treat certain types of chronic constipation. It works to increase fluid secretion in the intestines, which helps soften stool and promote regular bowel movements.
Brand Name:
Amitiza®
Pharmacologic Class:
Chloride channel activator
Laxative (secretagogue)
Mechanism of Action:
Lubiprostone specifically activates the ClC-2 chloride channels located on the apical (luminal) side of intestinal epithelial cells.
By activating these chloride channels, Lubiprostone:
Increases chloride-rich intestinal fluid secretion
Enhances intestinal motility
Facilitates passage of stool
Importantly, it works without significant changes in serum electrolyte levels, and it does not exert its effects through nervous system stimulation.
Indications (Uses):
Lubiprostone is FDA-approved for the treatment of:
Chronic Idiopathic Constipation (CIC) in adults
Irritable Bowel Syndrome with Constipation (IBS-C) in adult women
Opioid-Induced Constipation (OIC) in adults with chronic non-cancer pain
Note:
"Idiopathic” means of unknown cause.
Not indicated for children.
Dosage and Administration:
Dose depends on the condition being treated:
A. Chronic Idiopathic Constipation (CIC):
24 mcg orally twice daily with food and water
B. Opioid-Induced Constipation (OIC):
24 mcg orally twice daily with food and water
C. Irritable Bowel Syndrome with Constipation (IBS-C):
8 mcg orally twice daily with food and water
Administration Tips:
Take with food and water to reduce nausea
Swallow capsule whole (do not chew or break)
Discontinue use if severe diarrhea or cramping occurs
Available Dosage Forms and Strengths:
Soft gelatin capsules:
• 8 mcg
• 24 mcg
Side Effects:
Most common side effects:
Nausea (most frequent)
Diarrhea
Abdominal pain
Bloating
Headache
Flatulence
Less common, but serious adverse effects:
Hypotension and syncope (rare)
Dyspnea (shortness of breath, often within an hour of first dose, usually short-lived)
Generally, side effects are more common at the higher 24 mcg dose.
Warnings and Precautions:
Avoid use in patients with known mechanical gastrointestinal obstruction
Use with caution in patients with severe diarrhea or gastrointestinal disorders
Dosage adjustment may be considered in hepatic impairment (see below)
Dyspnea has been reported shortly after the first dose; typically short in duration and self-resolving
Contraindications:
Known or suspected mechanical gastrointestinal obstruction
Hypersensitivity to Lubiprostone or any ingredient in the formulation
Use in Pregnancy and Breastfeeding:
Pregnancy:
Category C (risk cannot be ruled out based on animal studies)
Use only if clearly needed; consult with a healthcare provider
Females of childbearing potential:
A negative pregnancy test is recommended before starting
Effective contraception is advised during treatment
Breastfeeding:
Unknown whether Lubiprostone is excreted in human breast milk
Use caution when administering to nursing mothers
Use in Special Populations:
Pediatric use: Safety and effectiveness not established
Geriatric use: No significant differences noted, but monitor closely
Hepatic impairment:
• Moderate impairment (Child-Pugh B): Reduce dose
• Severe impairment (Child-Pugh C): Further monitoring and caution advised
Drug Interactions:
Lubiprostone has minimal drug-drug interactions because it is locally acting in the gastrointestinal tract and has a low systemic absorption
However, caution is recommended when used with medications that cause diarrhea or dehydration
Storage:
Store at room temperature: 20°C to 25°C (68°F to 77°F)
Protect from moisture; keep capsules tightly closed in original packaging
Advantages of Lubiprostone:
✅ Offers a non-laxative, mechanism-based option for chronic constipation
✅ Not dependent on opioid receptors or the central nervous system
✅ Has a relatively low risk of dependence or electrolyte disturbance
✅ Can be effective in hard-to-treat constipation cases like OIC
Limitations & Considerations:
Higher incidence of nausea compared to placebo, especially in women
Not approved for children
More expensive than traditional laxatives
Requires food and water for better tolerability
Summary:
Lubiprostone (Amitiza) is a chloride channel activator used to treat chronic idiopathic constipation, opioid-induced constipation, and IBS with constipation (in women). It works by drawing fluid into the intestines to help ease stool passage. It is generally safe and well tolerated, though some people may experience nausea or other gastrointestinal side effects. Lubiprostone offers a more targeted mechanism than many other laxatives and is especially helpful when standard therapies are ineffective.
