A semisynthetic, water-soluble derivative of camptothecin (a natural alkaloid). It is an antineoplastic agent that appears as a white to yellow crystalline powder . It functions as a prodrug, requiring conversion to its active metabolite, SN-38.

Mechanism of Action

As a prodrug, it is converted by carboxylesterase enzymes to its active metabolite SN-38. SN-38 is a potent inhibitor of the enzyme DNA topoisomerase I. By stabilizing the topoisomerase I-DNA complex, it prevents DNA from unwinding, leading to DNA strand breaks that inhibit DNA replication and trigger apoptotic cancer cell death.

Applications

Primarily used in oncology:

Colorectal Cancer: Used alone or in combination with other agents for colon and rectal cancer that has metastasized or recurred after prior treatment .

Other Cancers: Also studied and used for lung, cervical, ovarian, and gastric cancers, and pancreatic adenocarcinoma.

Safety and Side Effects

Common and potentially severe side effects include:

Very Common (affecting >1 in 10): Nausea, vomiting, diarrhea, decreased appetite, alopecia (reversible hair loss), and low blood cell counts (neutropenia, anemia) .

Key Severe Risks: Late-onset diarrhea can be life-threatening if untreated; severe neutropenia increases infection risk. Patients with certain UGT1A1 genetic variants are at higher risk for severe neutropenia .

Other: Acute cholinergic syndrome (during or shortly after infusion), abdominal pain, fever, and fatigue.
