Xylometazoline hydrochloride is a topical nasal decongestant primarily used for the temporary relief of nasal congestion caused by conditions such as the common cold, hay fever, sinusitis, and other respiratory allergies.

Mechanism of Action

Xylometazoline acts as a selective agonist at alpha-adrenergic receptors, especially α2B-adrenoceptors, located on the blood vessels of the nasal mucosa. By stimulating these receptors, it causes vasoconstriction of the venous sinusoids and smaller arteries in the nasal submucosa. This vasoconstriction reduces blood flow and swelling in the nasal tissues, thereby decreasing nasal resistance and congestion and improving airflow. The onset of action is rapid, typically within 5 to 10 minutes, and the effect lasts approximately 6 to 8 hours. Compared to similar agents like oxymetazoline, xylometazoline has a slightly faster onset but a comparable duration of action.

Applications

Temporary relief of nasal congestion due to colds, allergic rhinitis, sinusitis, and other upper respiratory tract conditions.
Available as nasal sprays or drops for intranasal administration.

Routes of Administration

Intranasal application as sprays or drops, usually 1 to 3 sprays or drops per nostril every 8 to 10 hours as needed.
Maximum recommended use is typically 3 doses per day and no longer than 3 consecutive days to avoid rebound congestion.

Safety and Side Effects

Common side effects include nasal dryness, burning, stinging, sneezing, and runny nose.
Prolonged or excessive use can cause rhinitis medicamentosa (rebound nasal congestion) due to receptor downregulation and mucosal changes.
Rare but serious systemic side effects may occur if absorbed in significant amounts, including headache, dizziness, nervousness, palpitations, blurred vision, and hypertension.
Allergic reactions such as rash, swelling, or difficulty breathing require immediate medical attention.
Avoid use in patients with uncontrolled hypertension or cardiovascular diseases due to sympathomimetic effects.
