Pilocarpine hydrochloride is a parasympathomimetic alkaloid derived from the Pilocarpus plant, primarily used in ophthalmology and oral therapy. As a cholinergic agonist, it stimulates muscarinic receptors, leading to increased secretions and smooth muscle contraction. It is available in ophthalmic solutions (eye drops), gels, and oral tablets, with key applications including glaucoma treatment, xerostomia (dry mouth) management, and diagnostic tests for sweat secretion (e.g., cystic fibrosis).

Mechanism of Action

Pilocarpine binds to muscarinic (M3) receptors , mimicking acetylcholine to induce:

In the eye : Contraction of the ciliary muscle (accommodation) and iris sphincter (miosis), reducing intraocular pressure (IOP) by improving aqueous humor outflow.

Systemically : Stimulation of salivary, sweat, and lacrimal glands , increasing secretions.

On smooth muscle : Causes bronchoconstriction and gastrointestinal motility.

Dosage and Administration

Glaucoma (eye drops) : 1–2 drops of 1–4% solution up to 4 times daily (adjust per IOP response).

Xerostomia (oral tablets) : 5–10 mg taken 3–4 times/day, titrated to tolerance.

Diagnostic use : 5 mg orally for sweat testing.

Precautions:

Avoid touching dropper tip to prevent contamination.

Oral doses should be taken with food to reduce nausea.

Contraindicated in asthma, acute iritis, or narrow-angle glaucoma (unless pre-treated with iridotomy).

Adverse Reactions

Ophthalmic : Blurred vision, brow ache, miosis, conjunctival irritation; rarely retinal detachment.

Systemic (oral/inadvertent absorption) :
