Description	Cyclosporine is a lipophilic cyclic peptide compound isolated from the culture medium of certain Hyphomycetes fungi such as Cylindrocarpoluidum and Tolypocladium inflatum. In the 1980s, it was widely used as an immunosuppressant worldwide and is now used in various tissue and organ transplants to prevent rejection and treat immune related diseases.

Ciclosporin, also spelled cyclosporine and cyclosporin, is a calcineurin inhibitor, used as an immunosuppressant medication. It is taken orally or intravenously for rheumatoid arthritis, psoriasis, Crohn's disease, nephrotic syndrome, eczema, and in organ transplants to prevent rejection.

Applications

For treatment of leukemia, cancer, kidney transplantation, tuberculosis, etc.

Mechanism of action

Cyclosporine A has the ability to reversibly and selectively alter the function of T lymphocytes, prevent transcription of lymphokines genes, interfere with the transmission of primary information, inhibit the release of interleukin-2, interferon, and other immune factors, affect humoral and cellular immunity, and suppress the killing activity of NK cells; Inhibit the differentiation and proliferation of lymphocytes under antigen or mitogen stimulation. But it has no activity on the bone marrow, little effect on B cells, and no significant inhibitory effect on the survival of granulocytes at therapeutic doses. Oral digestion and absorption are slow and incomplete, only 4% to 60%, with an average of 30% of the administered dose; The relative bioavailability is 20% to 50%. The peak value is reached 2-8 hours after oral administration, with an average of 3.5 hours. Individual differences are significant, such as relative bioavailability of 5% -89% in kidney transplant patients and 8% -60% in liver transplant patients. This product is a fat soluble medication, and eating and consuming oily foods can increase absorption. Bile deficiency, stagnation, slow gastric emptying, increased gastrointestinal motility, pancreatic dysfunction, and steatosis all affect the absorption of this product.

CYCLOSPORIN A is an amide. Amides/imides react with azo and diazo compounds to generate toxic gases. Flammable gases are formed by the reaction of organic amides/imides with strong reducing agents. Amides are very weak bases (weaker than water). Imides are less basic yet and in fact react with strong bases to form salts. That is, they can react as acids. Mixing amides with dehydrating agents such as P2O5 or SOCl2 generates the corresponding nitrile. The combustion of these compounds generates mixed oxides of nitrogen (NOx).
