Description	Candesartan cilexetil is a prodrug of the potent, long-acting, and selective angiotensin II type 1 receptor (AT1) antagonist, candesartan. It is rapidly hydrolyzed to candesartan during gastrointestinal absorptio. Candesartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor in many tissues, such as vascular smooth muscle and the adrenal gland. Its action is, therefore, independent of the pathways for angiotensin II synthesis. Candesartan has much greater affinity ( > 10,000-fold) for the AT1 receptor than for the AT2 receptor.

Candesartan cilexetil is an oral angiotensin II receptor antagonist (ARB) used primarily for managing hypertension and heart failure. It acts as a prodrug, converting to its active metabolite candesartan in the gastrointestinal tract after administration.

Mechanism of Action

Blocks the binding of angiotensin II to type 1 (AT₁) receptors in blood vessels, kidneys, and other tissues.

Inhibits angiotensin II-induced vasoconstriction and sodium retention, leading to vasodilation and reduced blood volume.

Lowers blood pressure without affecting heart rate or causing reflex tachycardia.

Clinical Uses

Hypertension: Monotherapy or combination therapy with other antihypertensives (e.g., diuretics, calcium channel blockers).

Heart failure: Reduces morbidity and mortality in patients with symptomatic heart failure (NYHA classes II-IV) when added to standard therapy.

Diabetic nephropathy: Off-label use to slow renal function decline in patients with type 2 diabetes and hypertension.

Dosage & Administration

Available as tablets (4 mg, 8 mg, 16 mg, 32 mg) for oral use, usually taken once daily with or without food.

Hypertension: Initial dose 8 mg/day, titrated up to 32 mg/day based on blood pressure response.

Heart failure: Initial dose 4 mg/day, gradually increased to 32 mg/day as tolerated.

Dose adjustment required in patients with moderate-to-severe renal impairment or hepatic dysfunction.

Contraindications & Precautions

Contraindicated in patients with hypersensitivity to candesartan cilexetil, candesartan, or any excipients.

Avoid use in pregnancy (second/third trimesters) due to risk of fetal renal damage.

Use cautiously in patients with renal artery stenosis, hyperkalemia, or volume depletion (e.g., from diuretics).

Monitor serum potassium and creatinine levels regularly, especially in elderly patients or those with comorbidities.

Adverse Reactions

Common: Dizziness, headache, fatigue, diarrhea, and upper respiratory tract infections.

Rare: Hypotension (especially with initial dosing), hyperkalemia, renal dysfunction, and angioedema.

Discontinue immediately if angioedema (swelling of face, lips, tongue) or severe hypotension occurs.

Drug Interactions

Potassium-sparing diuretics/ACE inhibitors: Increased risk of hyperkalemia.

Candesartan cilexetil is a member of biphenyls.|Candesartan is an angiotensin-receptor blocker (ARB) that may be used alone or with other agents to treat hypertension. It is administered orally as the prodrug, candesartan cilexetil, which is rapidly converted to its active metabolite, candesartan, during absorption in the gastrointestinal tract. Candesartan lowers blood pressure by antagonizing the renin-angiotensin-aldosterone system (RAAS); it competes with angiotensin II for binding to the type-1 angiotensin II receptor (AT1) subtype and prevents the blood pressure increasing effects of angiotensin II. Unlike angiotensin-converting enzyme (ACE) inhibitors, ARBs do not have the adverse effect of dry cough. Candesartan may be used to treat hypertension, isolated systolic hypertension, left ventricular hypertrophy and diabetic nephropathy. It may also be used as an alternative agent for the treatment of heart failure, systolic dysfunction, myocardial infarction and coronary artery disease.
