    Description

Cladribine is a purine nucleoside antimetabolite analogue. Cladribine triphosphate, a phosphorylated metabolite of cladribine, incorporates into DNA, resulting in single-strand breaks in DNA, depletion of nicotinamide adenine dinucleotide (NAD) and adenosine triphosphate (ATP), and apoptosis. Because this agent is resistant to adenosine deaminase, an enzyme that inactivates some antineoplastic agents, it is selectively toxic to lymphocytes and monocytes which exhibit little deoxynucleotide deaminase activity.

    Cladribine More Info

Cladribine  is a synthetic purine nucleoside analog with potent antineoplastic and immunosuppressive properties, widely used in the treatment of certain hematological malignancies and autoimmune diseases.

    1. Chemical & Pharmacological Basics

    • Chemical Formula : \(C_{10}H_{12}ClN_5O_3\)
    • Mechanism of Action : Cladribine is incorporated into the DNA and RNA of rapidly dividing cells (e.g., cancer cells, immune cells). It inhibits ribonucleotide reductase and DNA polymerase, disrupting DNA synthesis and repair, ultimately leading to cell apoptosis. Unlike many chemotherapeutics, it is active against both dividing and resting lymphocytes, making it effective for immune-mediated disorders.

    2. Clinical Applications

    (1) Hematological Malignancies

    • Hairy Cell Leukemia (HCL) : Cladribine is a first-line treatment for HCL, with high response rates (over 90% overall response rate). It is administered as a single-agent therapy.
    • Chronic Lymphocytic Leukemia (CLL) : Used as a second-line treatment for patients with relapsed or refractory CLL, often in combination with other drugs.
    • Other Indications : May be used off-label for non-Hodgkin lymphoma and mycosis fungoides.

    (2) Autoimmune Diseases

    • Multiple Sclerosis (MS) : Approved in many countries for the treatment of relapsing-remitting multiple sclerosis (RRMS). It reduces the frequency of relapses and slows disease progression by selectively depleting pathogenic lymphocytes.

    3. Administration & Dosage

    • Route : Intravenous (IV) infusion or oral tablets (for MS).
    • Dosage Regimen : Varies by indication. For HCL, a standard IV dose is typically given over 7 consecutive days. For MS, oral dosing follows an intermittent schedule to minimize long-term immunosuppression.

    4. Safety Profile

    Common Adverse Effects

    • Myelosuppression : Leukopenia, neutropenia, thrombocytopenia (monitor blood counts regularly).
    • Infections : Increased risk of bacterial, viral, or fungal infections due to immunosuppression.
    • Gastrointestinal Symptoms : Nausea, vomiting, diarrhea, and stomatitis.

    Serious Warnings

    • Bone Marrow Toxicity : Prolonged or severe cytopenias may require dose adjustment or supportive care (e.g., growth factors).
    • Risk of Malignancy : Long-term use may increase the risk of secondary cancers (e.g., myelodysplastic syndrome, acute myeloid leukemia).
    • Contraindications : Hypersensitivity to cladribine, severe renal or hepatic impairment, and pregnancy (may cause fetal harm).

    5. Market & Supply Information

    • Cladribine is available as a generic drug in many regions, with brand-name formulations (e.g., Mavenclad for MS) still in use.
    • For pharmaceutical raw material suppliers, cladribine is produced under strict GMP standards, with purity requirements typically ≥99% for clinical use.
