Description	O-304 (O304) is a first-in-class, orally bioavailable pan-AMP-activated protein kinase (AMPK) activator that enhances AMPK signaling by suppressing PP2C-mediated dephosphorylation of AMPK Thr172, without directly inhibiting PP2C. In vitro, O-304 increased p-AMPK and downstream p-ACC in human Wi-38 fibroblasts and promoted AMPK-dependent, insulin-independent glucose uptake in skeletal L6 myotubes at 2.5–10 μM. In vivo, oral O-304 at 100 mg/kg/day for 8 weeks prevented high-fat-diet-induced hyperglycemia, hyperinsulinemia, and insulin resistance in diet-induced obese mice, while increasing skeletal-muscle p-AMPK; O-304 also increased muscle glucose uptake following 2 mg/g administration for 2 weeks. In hIAPP-transgenic obese mice, 100 mg/kg/day prevented glucose intolerance, hyperinsulinemia, and β-cell stress. These findings support O-304 as a research tool for studying AMPK signaling, glucose metabolism, insulin resistance, and metabolic disease.

O-304 (CAS No. 1261289-04-6) is a first-in-class, orally available pan-AMPK activator. It is a small-molecule, orally bioavailable compound that increases AMPK activity by suppressing the dephosphorylation of pAMPK.

Mechanism of Action

O-304 works by inhibiting Protein Phosphatase 2C (PP2C) — the enzyme that normally dephosphorylates and inactivates AMPK. By inhibiting PP2C, O-304 allows AMPK to remain phosphorylated at Thr172, maintaining it in its active form (p-AMPK). This activated p-AMPK then drives downstream pathways that result in increased glucose uptake and reduced β-cell stress. Additionally, O-304 functions as a PCSK9 modulator, reducing extracellular and secreted PCSK9 levels in human liver cells, and lowering PCSK9 plasma levels and total cholesterol in animal models.

Research Applications

O-304 exhibits significant potential for the treatment of type 2 diabetes (T2D) and associated cardiovascular complications. In preclinical in vivo studies, it increases glucose uptake in skeletal muscle, reduces β-cell stress, and promotes β-cell rest in diet-induced obese mice.

In a proof-of-concept Phase IIa clinical trial in type 2 diabetes patients on Metformin, O-304 reduced fasting plasma glucose levels and homeostasis model assessment of insulin resistance (HOMA-IR).

Other reported research activities include potential as an analgesic/AMPK alpha activator in peripheral settings and applications in treating conditions such as obesity, non-alcoholic steatohepatitis (NASH), and peripheral artery disease.
