Function and Efficacy
This product is a formylated derivative of the reduced form of folic acid, which is the activated form of folic acid in the body (see folic acid). The function of folic acid antagonists such as methotrexate is to bind to dihydrofolate reductase and block the conversion of folic acid to tetrahydrofolate. This product can directly provide the activated form of folic acid in the body, which can rescue the toxic reaction of excessive folic acid antagonists in the body, and is beneficial to the synthesis of thymine nucleotides, DNA, RNA and even proteins. This product can limit the degree of damage of methotrexate to normal cells, and through mutual competition, it can reverse the reaction of methotrexate to bone marrow and gastrointestinal mucosa, but it has no effect on the existing methotrexate neurotoxicity.

Indication
It is mainly used as an antidote for folic acid antagonists (such as methotrexate, pyrimethamine or trimethoprim). This product is often used clinically to prevent severe toxic effects caused by overdose or high-dose treatment of methotrexate. When oral folic acid is ineffective, it is also used for megaloblastic anemia caused by stomatitis, diarrhea, malnutrition, pregnancy or infancy, but it is not suitable for vitamin B12 deficiency anemia. In recent years, calcium folinate has been used as an adjuvant treatment for colon and rectal cancer, and combined with fluorouracil, it can prolong survival.
Usage and Dosage
1. Calcium folinate rescue therapy after high-dose methotrexate treatment: The dose of calcium folinate is determined according to the blood concentration of methotrexate. Generally, 24 hours after intravenous injection of methotrexate, calcium folinate 10 mg/m2 is given once every 6 hours for a total of 10 times. If gastrointestinal toxicity occurs, do not inject this product intrathecally. Guideline dosage for calcium folinate: Clinical research laboratory checks calcium folinate dosage and course of treatment. 24 hours after conventional elimination of methotrexate, plasma methotrexate levels are about 10 μmol, about 1 μmol after 48 hours, and less than 0.2 μmol after 72 hours. Within 60 hours, 15 mg is injected intramuscularly or intravenously once every 6 hours (starting 24 hours after the use of methotrexate, for a total of 10 times). 72 hours after the administration of methotrexate for delayed elimination of late methotrexate, the plasma methotrexate level is greater than 0.2 μmol, and is still greater than 0.05 μmol after 96 hours of medication. Continue to inject 15 mg intramuscularly or intravenously every 6 hours until the methotrexate level is less than 0.05 μmol. Early delayed elimination of methotrexate and/or acute kidney injury Plasma methotrexate level is greater than or equal to 50 μmol 24 hours after administration, or greater than or equal to 5 μmol 48 hours after administration, or serum creatinine increases by more than 10% in 24 hours after the use of methotrexate. Inject 150 mg intravenously every 3 hours until the methotrexate level is less than 1 μmol, and then inject 15 mg intravenously every 3 hours until the methotrexate level is less than 0.05 μmol. 2 When methotrexate is not eliminated smoothly or overdose is accidentally used: When methotrexate is accidentally overdosed, calcium folinate should be used as soon as possible for emergency treatment: When excretion is delayed, calcium folinate should also be used within 24 hours of methotrexate use. Generally, 10 mg of calcium folinate is injected intramuscularly or intravenously every 6 hours until the blood.
Adverse Reactions
Rarely, skin rash may occur. Look for allergic reactions such as hives or asthma.
Precautions
It is contraindicated in pernicious anemia and megaloblastic anemia caused by vitamin B12 deficiency.
Drug Interactions
The use of this product in large doses together with barbiturate, primidone or phenytoin sodium may affect the anti-epileptic effect.
