Protamine Sulfate Injection
Function and Efficacy
This product has a strong alkaline group, which can bind to the strongly acidic heparin in the body to form a stable complex. This direct antagonism causes heparin to lose its anticoagulant activity. Heparin binds to antithrombin III and strengthens its inhibitory effect on thrombin. Individual experiments have confirmed that this product can decompose the binding of heparin and antithrombin III, thereby eliminating its anticoagulant effect. This product also has a mild antithrombin kinase effect, but it is generally not used clinically to counteract the anticoagulant effect caused by non-heparin.
Ingredients
This product is a sterile aqueous solution of protamine sulfate and sodium chloride to make it isotonic.

Indication
Anti-heparin drugs. Used to treat bleeding caused by excessive heparin injection.
Usage and Dosage
Intravenous injection: To prevent heparin overdose, the dosage should be equivalent to the last heparin dosage (1 mg of protamine sulfate can neutralize 100 units of heparin). No more than 5 ml (50 mg) per time. Slow intravenous injection. Generally, the injection rate is 0.5 ml per minute, and the injection volume within 10 minutes should not exceed 50 mg. Since this product has its own anticoagulant effect, it should not exceed 100 mg within 2 hours (i.e., the effective duration of this product). Unless there is other solid evidence, the dose should not be increased.
Adverse Reactions
1. This product can cause bradycardia, chest tightness, dyspnea and hypotension, which are mostly caused by too rapid intravenous injection, which is caused by the direct effect of the drug on the myocardium or peripheral vasodilation; there are also reports of pulmonary hypertension or high blood pressure; 2. After injection, there are nausea, vomiting, flushing, hot flashes and fatigue. If the effect is short-lived, no treatment is required; 3. Allergies may occur occasionally.
Precautions
It is contraindicated for those who are allergic to this product.
Special Population Medication
Precautions for children: Intravenous drip for children: Anti-spontaneous bleeding, 5-8 mg/kg per day, divided into 2 times, 6 hours apart, each time diluted with 300-500 ml sterile saline, and half the dose after 3 days. The single dose should not exceed 25 mg; Intravenous injection: Anti-heparin overdose, the dosage should be equivalent to the last heparin dosage. Generally, a 1% solution is used, no more than 2.5 ml (25 mg) each time, and slowly injected intravenously. 1 mg of protamine sulfate can neutralize 100 units of heparin. Precautions for pregnancy and lactation: There is little information on the use of this product by pregnant and lactating women, so pregnant and lactating women should use it with caution. Precautions for the elderly: This experiment has not been conducted and there are no reliable references.
