Zolmitriptan Tablets
Function and Efficacy
Zolmitriptan shows high affinity to human recombinant 5-HT1D and 5-HT1B receptors, and moderate affinity to 5-HT1A receptors. The N-desmethyl metabolite of this product also shows high affinity to 5-HT1B/1D receptors, and moderate affinity to 5-HT1A receptors. It is currently believed that the onset of migraine is related to local cranial vasodilation and/or the release of sensory neuropeptides (vasoactive intestinal peptide, substance P, calcitonin gene-related peptide) from nerve endings of the trigeminal nervous system. The therapeutic effect of this product on migraine may be through the stimulating effect on the 5-HT1B/1D receptors of intracranial blood vessels (including arteriovenous anastomoses) and sensory nerves of the trigeminal nervous system, resulting in cranial vasoconstriction and inhibition of the release of post-inflammatory neuropeptides.

Appearance
This product is white or off-white tablets
Indication
This product is indicated for the acute treatment of migraine with or without aura. This product is not intended for the prevention of migraine, hemiplegic or basilar artery migraine. The safety and efficacy of this product for cluster headache has not been established.
Usage and Dosage
Oral. The starting dose of this product is 2.5 mg. If the headache recurs, it should be taken 2 hours after the last dose. Do not take more than 10 mg of this product within 24 hours.
Adverse Reactions
This product is well tolerated, and adverse reactions are mild, short-lived, and resolve on their own without treatment.
Precautions
It is contraindicated for patients who are allergic to any of the ingredients in this product. It should not be used by patients with uncontrolled blood pressure.
Special Population Medication
Precautions for children: The safety and effectiveness of this drug for children have not been determined. Precautions for pregnancy and lactation: There are currently no studies on the use of this drug in pregnant women. This drug should only be considered for use when the benefit to the mother outweighs the potential risk to the fetus. Mammalian studies have shown that zolmitriptan can enter human breast milk, but there is no data on human use. Therefore, breastfeeding women should use it with caution. Precautions for the elderly: The safety and effectiveness of this drug for patients over 65 years old have not been determined.
Drug Interactions
1. There is no evidence that the use of preventive migraine drugs (such as beta-blockers, oral dihydroergotamine, pizotifen) has any effect on the efficacy of this product. Acute symptomatic treatment such as the use of paracetamol, metoclopramide and ergotamine does not affect the pharmacokinetics and tolerance of this product. In healthy individuals, no pharmacokinetic interaction between this product and ergotamine has been found. This product is well tolerated when used in combination with ergotamine/caffeine. Compared with the use of this product alone, there is no increase in adverse reactions and no change in blood pressure. 2. Selegiline (a monoamine oxidase-β inhibitor) and chloroxetine (a selective 5-hydroxytryptamine reuptake inhibitor) have no effect on the pharmacokinetic parameters of this product. Other 5HT1D agonists should be avoided within 12 hours of treatment with this drug. After using moclobemide (a special monoamine chlorinase-A inhibitor), the area under the curve of zolmitriptan increased slightly (26%), and the area under the curve of the active metabolite increased threefold. Therefore, for patients using monoamine oxidase-A inhibitors, the recommended maximum dose of this product within 24 hours is 7.5 mg. 3. When used in combination with cimetidine and oral contraceptives, the blood concentration of this product can also be increased. When used in combination with propranolol, the metabolism of this product can be delayed.
