Function and Efficacy

The antibacterial spectrum of azithromycin is the same as other macrolide antibiotics. Its antibacterial effect on various staphylococci, streptococci, and pneumococci is slightly worse than that of erythromycin, and enterococci are resistant to this product; but this product is more effective than erythromycin against some Gram-negative bacteria, such as influenza bacilli, including beta-lactamase-producing strains, whose MIC is 8 times lower than that of erythromycin, about 0.06-1 mg/L. The MIC for Moracata is 0.03 mg/L, and its antibacterial effect on peptococci, peptostreptococci, Bacteroides, and Bacteroides fragilis is similar to or slightly worse than that of erythromycin. Its effect on intracellular pathogens such as mycoplasmas, chlamydia, and Legionella is also similar to that of erythromycin.

Appearance

This product is a white tablet or a film-coated tablet. After removing the coating, it is white or off-white. Each tablet is engraved with the words "ZTM250" and contains azithromycin dihydrate, equivalent to 0.25g of azithromycin.

Indication

It is mainly used for respiratory tract, skin and soft tissue infections caused by sensitive bacteria and sexually transmitted diseases caused by chlamydia. The effective rate for acute bronchitis, chronic obstructive pulmonary disease and pneumonia caused by influenza bacilli, pneumococcus and Moracata is 90%, and the bacterial clearance rate is 85%. For furuncle, cellulitis, etc. caused by Streptococcus pyogenes and Staphylococcus aureus, the cure rate is 53% to 74% and the effective rate is more than 90% with a 5-day therapy (total 1.5g) of this product once a day, and its efficacy is similar to that of oxacillin, erythromycin and cephalexin. A single dose of 1g is used to treat chlamydia infection, and its efficacy is similar to that of ciprofloxacin or doxycycline.

Usage and Dosage

Take this product at least 1 hour before or 2 hours after eating. Adults take 0.5g daily, once a day, for a course of 3 days; or 0.5g for the first dose, and 0.25g daily thereafter, for a course of 5 days. Single-dose 1g therapy can be used for sexually transmitted diseases. Elderly people can be given the adult dose. Children over 6 months old (under 45kg) take a daily dose of 10mg/kg, and the dosage and course of treatment are the same as those for adults. For example, if you take azithromycin suspension, 3-7 years old (15-25kg) take 5ml (200mg), 8-11 years old (26-35kg) take 7.5ml (300mg), 12-14 years old (36-45kg) take 10ml (400mg).

Adverse Reactions

1 The main adverse reactions are gastrointestinal symptoms such as abdominal pain, nausea, vomiting and diarrhea, with an incidence of about 9.6%; about 0.3% of treatment was discontinued due to adverse reactions, followed by skin rashes, etc. Most of them were mild and moderate, and there was no need to stop the drug. Occasionally, a temporary increase in transaminases was seen, and transient neutropenia was seen, but whether it was related to azithromycin has not been confirmed. 2 Like other macrolides such as erythromycin, severe allergic reactions such as neuroangioedema and anaphylactic shock are rare, but attention should still be paid. After taking the drug, attention should be paid to secondary infections caused by resistant bacteria such as fungi. 3 Central and peripheral nervous system reactions were 1.3%, and other skin rashes were 1%. Precautions

It is contraindicated in patients who are allergic to macrolide drugs.

Special Population Medication

Precautions for children: Regardless of the type of infection, it is recommended that the total dose of azithromycin in children should not exceed 1500 mg. Azithromycin tablets are only suitable for children weighing more than 45 kg, and the usage and dosage are the same as those for adults. Pediatric pharmacokinetic data suggest that the 20 mg/kg dose for children is equivalent to the 1200 mg dose for adults, but its Cmax value is higher. Precautions for pregnancy and lactation: Animal reproductive studies have shown that azithromycin can cross the placenta, but there is no sign of damage to the fetus. There is no data on the secretion of this product in breast milk and its safety for use in pregnant and lactating people, so this product can only be used when pregnant or lactating women have no appropriate choice. Precautions for the elderly: The method of administration and dosage are the same as those for adults.

Drug Interactions

This product has no interaction with theophylline, warfarin, carbamazepine, methylprednisolone, etc. This product can only be taken 1 hour before or 2 hours after taking antacids. A single dose of cimetidine 2 hours before taking this product will not affect the blood concentration of this product. Some macrolides can affect digoxin metabolism, so continuous use of this product may increase the blood concentration of digoxin, which should be noted. This product should not be used in combination with ergot drugs because it can cause ergot poisoning in patients; in addition, macrolides have been reported to increase the blood concentration of digoxin (changing intestinal flora), cyclosporine and carbamazepine (by inhibiting metabolism), so when AM is taken at the same time as these three drugs, the blood concentration of these drugs may also increase. The blood concentration of the latter needs to be monitored when used in combination.
