Sulfadiazine Suspension

Function and Efficacy

This product belongs to the sulfonamide broad-spectrum antibacterial drug, which has antibacterial effects on both Gram-positive and Gram-negative bacteria. However, bacterial resistance to this type of drug is common at present. The number of resistant strains in Staphylococcus, gonococci, meningococci, and Enterobacteriaceae has increased. Sulfonamides are also active against the following microorganisms: Chlamydia trachomatis, Nocardia asteroides, Plasmodium falciparum, and Toxoplasma gondii. Sulfonamides are broad-spectrum antibacterial agents, similar in structure to para-aminobenzoic acid (PABA), and can compete with PABA for dihydrofolate synthase in bacteria, thereby preventing PABA from being used as a raw material to synthesize folic acid required by bacteria, reducing the amount of metabolically active tetrahydrofolate, which is an essential substance for bacteria to synthesize purine, thymidine, and deoxyribonucleic acid (DNA), thereby inhibiting the growth and reproduction of bacteria. The effects of sulfonamides can be antagonized by PABA and its derivatives (procaine, tetracaine). In addition, the presence of pus and tissue decomposition products also has an antagonistic effect because it provides substances necessary for bacterial growth.

Appearance

This product is a suspension aqueous solution of fine particles, which will precipitate slightly after standing and become a uniform white suspension after shaking.

Indication

Used for infections such as hemolytic streptococci, meningococci, and pneumococci.

Usage and Dosage

Oral administration, 10 ml at a time, twice a day. For the treatment of meningitis, 10 ml at a time, 4 times a day. Shake well before taking.

Adverse Reactions

1. Allergic reactions are relatively common. It can manifest as drug rash. In severe cases, exudative erythema multiforme, exfoliative dermatitis and bullous epidermolysis atrophic dermatitis may occur; there are also serum sickness-like reactions such as photosensitivity reaction, drug fever, joint and muscle pain, fever, etc. Anaphylactic shock is occasionally seen. 2. Neutropenia or deficiency, thrombocytopenia, and occasionally aplastic anemia. Patients may show symptoms of sore throat, fever and bleeding tendency. 3. Hemolytic anemia and hemoglobinuria. This is prone to occur in patients who lack glucose-6-phosphate dehydrogenase after using sulfonamides, and is more common in newborns and children than in adults. 4. Hyperbilirubinemia and neonatal kernicterus. Because this product competes with bilirubin for protein binding sites, it can cause an increase in free bilirubin. Newborns have imperfect liver function,

Precautions

It is contraindicated for those who are allergic to sulfonamides, newborns and infants under two months old, and those with poor liver and kidney function.

Special Population Medication

Precautions for children: Since sulfonamides can compete with bilirubin for binding sites on plasma proteins, and the acetyltransferase system of newborns is not fully developed, the free blood concentration of sulfonamides increases, and inhibition increases the risk of kernicterus. Therefore, this type of drug is contraindicated in newborns and infants under 2 months old. Precautions for pregnancy and lactation: 1. This product can cross the blood-placental barrier to the fetus, and animal experiments have found that it has teratogenic effects. There is a lack of sufficient data in human studies, so pregnant women should avoid using it. 2. This product can be secreted in breast milk, and the concentration in breast milk can reach about 50% to 100% of the maternal blood drug concentration. The drug may have an impact on infants. The use of this product in newborns with glucose-6-phosphate dehydrogenase deficiency may cause hemolytic anemia. In view of the above reasons, this product should not be used by lactating women. Precautions for the elderly: Elderly patients have an increased chance of serious adverse reactions when using this product. For example, severe rash, bone marrow suppression and thrombocytopenia are common serious adverse reactions in the elderly. Therefore, elderly patients should avoid using it, and when there is an indication, the pros and cons should be weighed before making a decision.
