Function and Efficacy

This product is a virus inhibitor. It can non-competitively block the phosphate binding site of viral DNA polymerase, prevent the separation of pyrophosphate from triphosphonate deoxynucleoside and the extension of viral DNA chain. This product does not rely on the activation of viral thymidine kinase in cells, and viral replication can still be restored after discontinuation of this product. In vitro tests show that this product can inhibit the replication of all herpes viruses, including herpes simplex (HSV-1 and HSV-2), herpes zoster, EB virus, human herpes virus-6 and cytomegalovirus. This product can also non-competitively inhibit HIV reverse transcriptase and hepatitis B virus DNA polymerase.

Appearance

This product is a colorless clear liquid.

Indication

This product is mainly used to treat cytomegalovirus retinitis in immunocompromised people (such as AIDS patients). It can also be used to treat skin and mucosal herpes simplex virus infection or herpes zoster virus infection in immunocompromised people (such as HIV-infected patients) who are resistant to acyclovir.

Usage and Dosage

Intravenous drip. 1 Cytomegalovirus retinitis (1) During the induction period, the medication is administered once every 8 hours, 60 mg/kg of body weight is dripped once, and the drip is infused for more than 1 hour by an infusion pump for 14 to 21 consecutive days. Depending on the effect after treatment, it can also be administered once every 12 hours at 90 mg/kg of body weight. (2) During the maintenance period, the medication is administered once every day at 90 mg/kg of body weight. The drip is infused for more than 2 hours by an infusion pump. If the patient's symptoms of retinitis worsen during the maintenance period, the induction dose should be restored. 2 Herpes simplex and herpes zoster: 40 mg/kg of body weight is administered once every 8 hours, and the drip is infused for 1 hour by an infusion pump for a total of 14 to 21 days. The dose should be adjusted for patients with creatinine clearance lower than 96 ml/min.

Adverse Reactions

1. Renal function impairment is the most important adverse reaction of this product, which can cause acute tubular necrosis, nephrogenic diabetes insipidus and sodium foscarnet crystalluria. There may also be hypocalcemia or hypercalcemia, high or low blood phosphorus, hypokalemia, etc. 2. Central nervous system symptoms: headache, tremor, irritability, hallucinations, convulsions, etc., which may be related to electrolyte disorders. 3. Blood system: anemia, granulocytopenia, thrombocytopenia, etc. 4. Metabolic and nutritional disorders: hyponatremia and lower limb edema, elevated lactate dehydrogenase, alkaline phosphatase or amylase. 5. Cardiovascular system: ECG abnormalities, hypertension or hypotension, ventricular arrhythmias. 6. Other reactions: nausea, vomiting, loss of appetite, abdominal pain, fever, abnormal liver function and phlebitis, etc.

Precautions

This product is contraindicated in patients who are allergic to it or whose creatinine clearance is lower than 0.4ml/(minkg).

Special Population Medication

Precautions for children: There is no experience of using this product for children. Precautions for pregnancy and lactation: Pregnant women should not use this product unless necessary. Lactating women should stop breastfeeding while using this product. Precautions for the elderly: No such test has been conducted and there is no reliable reference literature.

Drug Interactions

1. This product may increase renal toxicity when used in combination with other nephrotoxic drugs such as aminoglycoside antibiotics and amphotericin B. 2. When used in combination with pentamidine injection (intravenous), there may be a risk of anemia. It may cause hypocalcemia, hypomagnesemia and renal toxicity. 3. When used in combination with zivdodine, anemia may be aggravated, but no aggravation of bone marrow suppression has been found.
