Function and Efficacy

Some unsaturated fatty acids cannot be synthesized by the body itself and need to be taken from vegetable oils. They are indispensable nutrients for the body, so they are called essential fatty acids. They are also the precursors of physiologically active substances such as prostaglandins, thromboxanes and leukotrienes. Fatty acids are the main energy source for the human body, and fatty acid oxidation is an important source of energy in the body. When the oxygen supply is sufficient, fatty acids can be decomposed into CO2 and H2O in the body and release a large amount of energy, which is used by the body in the form of ATP. Except for brain tissue, most tissues can oxidize fatty acids, especially liver and muscle. Phospholipids are the basic skeleton of the lipid bilayer of cell biological membranes (cell membranes, nuclear membranes, mitochondrial membranes), and are also the main components of various lipoproteins, participating in the transportation of fat and cholesterol. If the phospholipids in plasma are too low, the cholesterol/phosphatidylcholine ratio will increase, and cholesterol deposition will easily occur, causing atherosclerosis. Therefore, phospholipids have the effect of resisting hypercholesterolemia. In addition, in bile, phospholipids form micelles together with bile salts and cholesterol, which is conducive to the dissolution and excretion of cholesterol. Phospholipids are widely distributed in nature. They are abundant in seeds such as soybeans in plants, and are highest in nerve tissues in animals. This product is a sterilized fat emulsion for intravenous infusion, containing soybean oil for injection and lecithin for injection, of which about 60% of the fatty acids are essential fatty acids. The particle size and biological characteristics of this product are similar to those of natural chylomicrons.

Indication

Energy supplement. This product is one of the components of intravenous nutrition, providing energy and essential fatty acids for the body. It is used to supplement energy and essential fatty acids in parenteral nutrition, prevent and treat essential fatty acid deficiency in the human body, and provide essential fatty acids for patients who cannot maintain and restore normal essential fatty acid levels through oral administration. 30% fat emulsion injection is more suitable for patients with limited infusion volume and highly increased energy requirements. Lecithin can assist in the treatment of atherosclerosis, fatty liver, pediatric eczema, and neurasthenia. It is used as a solubilizer, emulsifier, and antioxidant for oils and fats in pharmaceutical excipients.

Usage and Dosage

Adults: intravenous drip, based on fat mass, the maximum recommended daily dose is 3g (triglycerides)/kg. The energy provided by this product can account for 70% of the total energy. The infusion time of 500ml of 10% and 20% fat emulsion injection is not less than 5 hours; the infusion time of 250ml of 30% fat emulsion injection is not less than 4 hours. Newborns and infants: The daily dose of 10% and 20% fat emulsion injection is 0.5-4g (triglycerides)/kg, and the infusion rate does not exceed 0.17g/(kg·hour). The maximum daily dose should not exceed 4g/kg. Only when serum triglycerides, liver function, oxygen saturation and other indicators are closely monitored can the infusion dose be gradually increased to 4g/kg per day. For premature infants and low-weight newborns, it is best to infuse continuously for 24 hours, starting with a daily dose of 0.5-1g/kg, and then gradually increasing to 2g/kg per day. Essential fatty acid deficiency: To prevent and treat essential fatty acid deficiency (EFAD), at least 4% to 8% of the energy in non-protein calories should be provided by fat emulsion injection to supply sufficient amounts of linoleic acid and linolenic acid. When EFAD is combined with stress, the amount of fat emulsion injection required for the treatment of EFAD should also be increased accordingly. Usage: This product can be infused alone or used to prepare an "all-in-one" nutrient mixture containing glucose, fat, amino acids, electrolytes, vitamins and trace elements. Other drugs can only be added to this product if compatibility is guaranteed. This product can also be mixed with glucose injection or amino acid injection through a Y-type tube and then injected into the body. This method is applicable to both central veins and peripheral veins. Under aseptic operating conditions, the following drugs can be added to this product. 1. Adverse Reactions

It can cause fever, occasionally chills, nausea and vomiting. Other side effects are relatively rare, including: 1. Immediate and early side effects: hypersensitivity reactions (allergic reactions, rashes, urticaria), respiratory effects (such as shortness of breath) and circulatory effects (such as hypertension/hypotension). Hemolysis, reticulocytosis, abdominal pain, headache, fatigue, abnormal penile erection, etc. 2. Delayed side effects: Long-term infusion of this product may cause thrombocytopenia in infants. In addition, even if this product is not used during long-term intravenous nutrition, there will be temporary abnormalities in liver function indicators. Phlebitis, vascular pain and bleeding tendency may occasionally occur. 3. When the patient's fat clearance ability is reduced, fat overload syndrome may occur despite normal infusion rate. Fat overload syndrome may occasionally occur in patients with renal dysfunction and infection. Fat overload syndrome is manifested as: hyperlipidemia, fever, fat infiltration, organ dysfunction, etc., but generally the above symptoms will subside as long as the infusion is stopped.
