Function and Efficacy

Phenformin hydrochloride is an oral hypoglycemic drug of the biguanide class. This product does not stimulate the secretion of insulin by B cells, and there is no significant change in the insulin concentration in the blood after taking the drug. The mechanism of action of this product in lowering blood sugar may be: ① Increase the sensitivity of peripheral tissues to insulin and increase insulin-mediated glucose utilization; ② Increase the utilization of glucose by non-insulin-dependent tissues, such as the brain, blood cells, renal medulla, intestines, skin, etc.; ③ Inhibit hepatic gluconeogenesis and reduce hepatic glucose output; ④ Inhibit the uptake of glucose by intestinal wall cells; ⑤ Inhibit the biosynthesis and storage of cholesterol and reduce the levels of blood triglycerides and total cholesterol. Unlike the action of insulin, this product has no effect on promoting fat synthesis, has no obvious hypoglycemic effect on normal people, and generally does not cause hypoglycemia when used alone for type 2 diabetes.

Indication

It is used for type 2 diabetes patients who are not satisfied with diet control alone, especially obese patients and patients with hyperinsulinemia. This product not only has the effect of lowering blood sugar, but also may help to reduce weight and hyperinsulinemia. For some diabetic patients who have poor response to sulfonylurea treatment, this product combined with sulfonylurea hypoglycemic drugs can produce a synergistic effect, which is better than the effect of using each alone.

Usage and Dosage

The dosage should be in accordance with the doctor's advice, and the principle of personalized administration should be adopted. 1. Dosage method for single treatment: When starting treatment, generally take orally once a day, 25 mg each time, before meals; after a few days, the number of doses can be increased to 2-3 times, 25 mg each time; 2. When used in combination with sulfonylurea drugs: once a day in the first week, 25 mg each time, before meals; after testing blood sugar in the second week, the number of doses per day can be gradually increased to 2-3 times a day, 25 mg each time, until the blood sugar level drops to or approaches normal values. The maximum daily oral dose of this product is generally not more than 75 mg, otherwise hyperlactatemia or lactic acidosis is likely to occur; in order to reduce gastrointestinal side effects, this product should be taken with food.

Adverse Reactions

1 Common symptoms include: nausea, vomiting, diarrhea, metallic taste in the mouth; 2 There may be weakness, tiredness, weight loss, dizziness, and rash; 3 Lactic acidosis may also occur, with clinical manifestations of vomiting, abdominal pain, hyperventilation, and mental disorders. The increased lactic acid concentration in the blood cannot be explained by uremia, ketoacidosis, or salicylic acid poisoning; 4 It can reduce the intestinal absorption of vitamin B12, reduce hemoglobin, produce megaloblastic anemia, and can also cause malabsorption.

Precautions

1. Type II diabetes mellitus accompanied by ketoacidosis, liver and kidney dysfunction (serum creatinine exceeds 1.5 ml/dl), heart failure, acute myocardial infarction, severe infection and trauma, major surgery, and clinical hypotension and hypoxia; 2. Special Population Medication

Precautions for children: Not clear yet. Precautions for pregnancy and lactation: Pregnant women and lactating women should not use this product. Precautions for the elderly: Since the liver and kidney functions of elderly patients may be reduced, the lowest possible maintenance dose should be selected; since this product is mainly excreted through the kidneys, the patient's renal function should be fully assessed before adjusting the patient's dosage. Elderly patients should avoid taking this product at high doses.

Drug Interactions

1. When used in combination with insulin, the hypoglycemic effect is enhanced and the insulin dose should be reduced. 2. This product can enhance the anticoagulant effect of anticoagulants (such as warfarin) and may cause bleeding tendency.
