Diltiazem hydrochloride sustained-release pellets

Function and Efficacy

This product is a calcium channel blocker, and its action is related to the inhibition of calcium ion influx during myocardial or vascular smooth muscle membrane depolarization. Anti-anginal effect and mechanism: This product can dilate the epicardial and endocardial coronary arteries, relieve angina pectoris caused by spontaneous or ergonovine-induced coronary artery spasm; in exertional angina pectoris, this product dilates peripheral blood vessels, lowers blood pressure, and reduces cardiac workload, thereby reducing oxygen demand, improving the double product of systolic blood pressure and heart rate, increasing exercise tolerance and relieving exertional angina pectoris. Used to treat hypertension, this product relaxes vascular smooth muscle, reduces peripheral vascular resistance, and lowers blood pressure, and is not accompanied by reflex tachycardia. This product's inhibition of slow calcium channels in myocardial cells reduces the autonomy and conductivity of the sinoatrial node and atrioventricular node, so it is used to treat supraventricular tachyarrhythmias. Since this product can improve left ventricular diastolic function, it can also be used to treat hypertrophic cardiomyopathy.

Appearance

This product is white or off-white spherical pellets.

Indication

Treat angina pectoris and hypertension. Angina pectoris caused by coronary artery spasm, including resting angina pectoris or variant angina pectoris, or exertional angina pectoris caused by coronary artery obstruction. It can also be used to treat supraventricular tachyarrhythmias and hypertrophic cardiomyopathy.

Usage and Dosage

oral

Adverse Reactions

1. Placebo-controlled trials were conducted abroad for the treatment of angina pectoris, and the results showed that this product did not have more adverse reactions than placebo. 2. The most common adverse reactions and incidence rates observed in clinical treatment of angina pectoris patients are: edema, headache, nausea, dizziness, rash, and weakness. 3. Uncommon adverse reactions (<1%) include the following: 1. Cardiovascular system: angina pectoris, arrhythmia, atrioventricular block (I, II, III degree), bradycardia, bundle branch block, congestive heart failure, abnormal electrocardiogram, hypotension, palpitations, syncope, tachycardia, and ventricular premature beats. (2.) Nervous system: dreaming, amnesia, depression, abnormal gait, hallucinations, insomnia, nervousness, paresthesia, personality changes, drowsiness, and tremors. 3. Digestive system: anorexia, constipation, diarrhea

Precautions

1. Sick sinus syndrome. 2. II or III degree atrioventricular block (except for the above two cases where a ventricular pacemaker is placed). 3. Hypotension <12kPa (<90mmHg). 4. Allergic to this product. 5. Acute myocardial infarction and pulmonary congestion.

Drug Interactions

1. When using drugs that affect cardiac contraction and (or) conduction at the same time, due to the possible synergistic effect, the use of this product must be cautious and the dosage must be carefully titrated. It is biotransformed by P450 cytochrome oxidase in the body. The simultaneous use of this product and other drugs through the same biotransformation pathway can lead to competitive inhibition of metabolism, so caution should be exercised when using multiple drugs. When starting or stopping simultaneous use, the dosage of drugs with the same metabolism, especially drugs with low therapeutic index or patients with impaired liver and kidney function, must be adjusted to maintain a reasonable blood drug concentration. 2. Beta-blockers may affect cardiac conduction, especially in patients with sick sinus syndrome or atrioventricular conduction block, and in patients with impaired left ventricular function, they can affect ventricular function, all of which have synergistic effects. Propranolol can increase the bioavailability of this product by nearly 50%, so the dose of propranolol must be adjusted. 3. Cimetidine increases the area under the drug-time curve of this product's blood concentration due to the inhibition of P450 cytochrome, so the dosage of this product needs to be adjusted. Although ranitidine can increase the blood concentration of this product, it is not obvious. 4 This product can increase the blood concentration of digoxin by 20%, but there are also reports that it has no effect. Although the results are contradictory, the blood concentration of digoxin should be monitored when starting to adjust and stop treatment with this product to avoid excessive or insufficient digitalis. 5 Anesthetics have inhibitory effects on myocardial contraction, conduction, and automaticity, and have vasodilation effects, and have synergistic effects with calcium channel blockers. Therefore, when these two drugs are used at the same time, the dosage must be carefully titrated.
