Nitroglycerin Injection

Function and Efficacy

The main pharmacological action is to relax vascular smooth muscle. Nitroglycerin releases nitric oxide (NO), which is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle and other tissues, leading to dephosphorylation of myosin light chain, regulating the contractile state of smooth muscle, and causing vasodilation.

Indication

It is used for the treatment and prevention of angina pectoris caused by coronary heart disease, and can also be used to lower blood pressure or treat congestive heart failure.

Usage and Dosage

Injection: Dilute with 5% glucose injection or sodium chloride injection and drip intravenously. The initial dose is 5μg/min. It is best to use an infusion pump to infuse at a constant rate. For lowering blood pressure or treating heart failure, increase by 5μg/min every 3 to 5 minutes. If it is ineffective at 20μg/min, increase by 10μg/min, and then 20μg/min. There are great individual differences in patients' response to this drug. There is no fixed suitable dose for intravenous drip. The dosage should be adjusted according to individual blood pressure, heart rate and other hemodynamic parameters.

Adverse Reactions

In the early stage of medication, nitrate-induced vasodilatory headache may occur, as well as facial flushing, dry mouth, dizziness, orthostatic hypotension and reflex tachycardia. Occasionally, blood pressure may drop significantly, bradycardia, angina pectoris may worsen and syncope may occur. Some patients may experience numbness in the oral mucosa.

Precautions

It is contraindicated in patients with early myocardial infarction (with severe hypotension and tachycardia), severe anemia, glaucoma, increased intracranial pressure, and known allergy to nitroglycerin. It is also contraindicated in patients using sildenafil citrate (Viagra), which enhances the antihypertensive effect of nitroglycerin.

Special Population Medication

Precautions for children: The safety and efficacy of the drug for children are still uncertain. Precautions for pregnancy and lactation: It is not known whether it will cause fetal harm or affect fertility, so it should only be used in pregnant women when it is necessary. It is also not known whether it is excreted from human breast milk, so pregnant women should be cautious when using it intravenously. Precautions for the elderly: It is not yet clear. Drug Interactions

The blood pressure lowering effect of this class of drugs can be enhanced when used in combination with other vasodilators, calcium antagonists, beta-blockers, antihypertensive drugs, tricyclic antidepressants and alcohol. Care should be taken to avoid hypotension.
