Phentolamine Mesylate Injection

1. Pharmacological action Phentolamine mesylate is a short-acting non-selective alpha-receptor (alpha 1, alpha 2) blocker that can antagonize the effects of adrenaline and norepinephrine in the blood circulation, dilate blood vessels and reduce peripheral vascular resistance; antagonize the effects of catecholamines and is used to diagnose and treat pheochromocytoma, but has little effect on the blood pressure of normal people or patients with essential hypertension; it can reduce peripheral vascular resistance, reduce cardiac afterload, reduce left ventricular end-diastolic pressure and pulmonary artery pressure, increase cardiac output, and can be used to treat heart failure. 2. Carcinogenicity, mutagenicity and reproductive toxicity Long-term tests have shown that phentolamine mesylate has no obvious carcinogenicity, mutagenicity and reproductive toxicity. In tests on rats, mice and rabbits, no teratogenicity or embryotoxicity of phentolamine mesylate was found.

This product is a colorless or slightly yellow clear liquid.

1. Used to diagnose pheochromocytoma and treat hypertension caused by it, including hypertension occurring during surgical resection. It can also be used to assist in the diagnosis of pheochromocytoma based on the blood pressure response to this product; 2. Treat left ventricular failure; 3. Treat the overflow of intravenous norepinephrine to prevent skin necrosis.

1 Common dosage for adults: (1) For phentolamine test, 5 mg is injected intravenously. 1 mg can also be injected first. If the reaction is negative, 5 mg is given. In this way, false positive results can be reduced and the risk of a sharp drop in blood pressure can be reduced; (2) For the prevention of skin necrosis, 10 mg of this product is added to every 1000 ml of norepinephrine solution for intravenous drip as a preventive measure. If norepinephrine overflow has occurred, 5-10 mg of this product is added to 10 ml of sodium chloride injection for local infiltration. This method is effective within 12 hours after the overflow; (3) For pheochromocytoma surgery, if blood pressure rises during the operation, 2-5 mg can be injected intravenously or dripped at 0.

The more common ones include orthostatic hypotension, tachycardia or arrhythmia, nasal congestion, nausea, vomiting, etc.; syncope and fatigue are less common; sudden chest pain (myocardial infarction), confusion, headache, ataxia, slurred speech, etc. are extremely rare.

It is contraindicated for patients with severe arteriosclerosis and renal insufficiency, hypotension, coronary heart disease, myocardial infarction, gastritis or gastric ulcer, as well as those who are allergic to this product.

Precautions for children: Precautions for pregnancy and lactation: Use with caution after weighing the pros and cons. Precautions for the elderly: The possibility of hypothermia in the elderly increases, so the dosage should be appropriately reduced.

1. When used in combination with sympathomimetic amines, the latter's peripheral vasoconstriction effect is offset or weakened; 2. When used in combination with guanethidine, the incidence of orthostatic hypotension or bradycardia increases; 3. When used in combination with diazoxide, the inhibitory effect of diazoxide on insulin release is inhibited; 4. Sedative hypnotics such as phenobarbital and doxepin and antihypertensive drugs such as reserpine and jiangyaling can enhance the antihypertensive effect of this product; 5. When used in combination with cardiac glycosides, its toxic reactions can be enhanced; 6. Avoid use with iron preparations; 7. This product cannot be used in combination with nitroglycerin drugs.
