Nimodipine Tablets
Function and Efficacy
Nimodipine is a Ca channel blocker. Under normal circumstances, the contraction of smooth muscle depends on the entry of Ca into the cell, which leads to the depolarization of transmembrane current. Nimodipine achieves the purpose of relieving vasospasm by effectively preventing Ca from entering the cell and inhibiting the contraction of smooth muscle. Animal experiments have shown that the effect of nimodipine on cerebral arteries is much stronger than that on arteries in other parts of the body, and because it has a high lipophilic characteristic, it is easy to pass through the blood-brain barrier. When used for the treatment of subarachnoid hemorrhage, the concentration in cerebrospinal fluid can reach 12.5ng/ml. It can be inferred that it can be used clinically to prevent vasospasm after subarachnoid hemorrhage, but the mechanism of action of this drug in human application is still unclear. In addition, it also has the effect of protecting and promoting memory and promoting intellectual recovery. Therefore, it can selectively act on cerebral vascular smooth muscle, dilate cerebral blood vessels, increase cerebral blood flow, and significantly reduce ischemic brain damage caused by vasospasm.

Appearance
Light yellow tablets, odorless and tasteless. Not stable to light, but relatively stable to moisture and heat.
Indication
Calcium channel blockers are used for ischemic cerebrovascular disease, migraine, cerebral vasospasm caused by mild subarachnoid hemorrhage, sudden deafness, and mild to moderate hypertension.
Usage and Dosage
1. Ischemic cerebrovascular disease: Oral. 30-120 mg per day, divided into 3 times, for one month. 2. Migraine: Oral. 40 mg per time, 3 times per day, 12 weeks as a course of treatment, the effective rate is 88%, about half of the cases can be basically cured or significantly effective, it can reduce the pain degree, frequency and duration of attacks, and prevent the appearance of premonitory symptoms for vascular, tension, cluster and mixed headaches. 3. Cerebral vasospasm caused by subarachnoid hemorrhage: Oral. 40-60 mg per time, 3-4 times per day, 3-4 weeks as a course of treatment. For patients who need surgery, stop taking the medicine on the day of surgery, and continue to take it later. 4. Sudden deafness: Oral. 40-60 mg per day, divided into 3 times, 5 days as a course of treatment, generally 3-4 courses of treatment. 5. Mild and moderate hypertension: patients with hypertension and cerebrovascular disease can be given priority. Oral. Start with 40 mg once, 3 times a day, the maximum daily dose is 0.24 g.
Adverse Reactions
A large number of clinical practices have shown that when patients with subarachnoid hemorrhage are treated with nimodipine, about 11.2% of patients experience adverse reactions. The most common adverse reactions are: (1) decreased blood pressure, the degree of which is related to the drug dose. (2) hepatitis. (3) skin irritation. (4) gastrointestinal bleeding. (5) thrombocytopenia. (6) occasional transient dizziness, headache, facial flushing, vomiting, gastrointestinal discomfort, etc. In addition, after oral administration of nimodipine, some patients may experience an increase in alkaline phosphatase (ALP), lactate dehydrogenase (LDH), AKP, an increase in blood sugar, and an increase in platelet count in some individuals.

Drug Interactions
(1) When used in combination with other calcium ion antagonists that act on the cardiovascular system, it can increase the efficacy of other calcium ion antagonists. (2) When nimodipine 90 mg/day is used in combination with cimetidine 1000 mg/day for more than one week, the blood concentration of nimodipine can increase by 50%, which may be related to the inhibition of nimodipine metabolism by cimetidine in the liver cytochrome P450
