Function and Efficacy
Nifedipine is a dihydropyridine calcium antagonist that selectively inhibits the transmembrane transport of calcium ions into myocardial cells and smooth muscle cells, and inhibits the release of calcium ions from cells without changing the plasma calcium ion concentration. 1. Pharmacological effects ① This product can simultaneously relax the coronary arteries in the normal blood supply area and the ischemic area, antagonize spontaneous or ergonovine-induced coronary artery spasm, increase the delivery of myocardial oxygen in patients with coronary artery spasm, and relieve and prevent coronary artery spasm. ② This product can inhibit myocardial contraction, reduce myocardial metabolism, and reduce myocardial oxygen consumption. ③ This product can relax peripheral resistance vessels, reduce peripheral resistance, and can reduce systolic and diastolic blood pressure, reducing cardiac afterload. ④ This product can delay the sinus node function and atrioventricular conduction of isolated hearts; electrophysiological studies of whole animals and humans have not found that this product has the effect of delaying atrioventricular conduction, prolonging the recovery time of the sinus node, and slowing down the sinus node rate. 2. Carcinogenicity, mutagenicity and reproductive toxicity No carcinogenic effect. No mutagenicity. High doses can reduce the fertility of female mice; can cause teratogenesis; can cause miscarriage (increased drug absorption rate in fetal mice, increased fetal mortality, and decreased survival rate of newborn mice). Pregnant monkeys taking 2/3 to 2 times the maximum human dose can cause small placentas and chorionic dysplasia; giving rats 3 times the maximum human dose can cause prolonged pregnancy. The impact on human fertility is still unclear.

Appearance

Indication
Various types of hypertension and angina pectoris.
Usage and Dosage
Oral: 10-20 mg once, twice a day. Maximum dose: 40 mg once, 0.12 g a day.
Adverse Reactions
1. Common peripheral edema after taking the drug (peripheral edema is related to the dose, the incidence is 4% when taking 60 mg a day, and 12.5% when taking 120 mg a day); dizziness; headache; nausea; fatigue and facial flushing (10%). Transient hypotension (5%), most of which do not require discontinuation of the drug (transient hypotension is related to the dose, the incidence is 2% when taking 60 mg a day, and the incidence is 5% when taking 120 mg a day). Some patients experience angina pectoris, which may be related to the hypotensive reaction. Palpitations; nasal congestion; chest tightness; shortness of breath; constipation; diarrhea; gastrointestinal cramps; abdominal distension; skeletal muscle inflammation; joint stiffness; muscle spasms; mental tension; tremors; nervousness; sleep disorders; blurred vision; imbalance, etc. (2%) can also be seen. Syncope (0.5%), which will no longer occur if the dose is reduced or used in combination with other anti-anginal drugs. 2. Rare anemia; leukopenia; thrombocytopenia; purpura; allergic hepatitis; gingival hyperplasia; depression; paranoia; instant blindness at peak blood concentration; erythromelalgia; antinuclear antibody positive arthritis, etc. (0.5%). 3. Possible serious adverse reactions: myocardial infarction and congestive heart failure occur in 4% of cases; pulmonary edema occurs in 2% of cases; arrhythmia and conduction block occur in less than 0.5% of cases each. 4. Allergic hepatitis, rash, and even exfoliative dermatitis may occur in patients allergic to this product.
Precautions
It is contraindicated in patients allergic to nifedipine.
Special Population Medication
Precautions for children: Not clear yet. Precautions for pregnancy and lactation: 1. No detailed clinical research data. Nifedipine is clinically used for pregnant women with hypertension. 2. Nifedipine can be secreted into breast milk, so lactating women should stop taking the drug or stop breastfeeding. Precautions for the elderly: The half-life of nifedipine is prolonged in the elderly, so pay attention to adjusting the dose when using it.
Drug Interactions
1. Nitrates are used in combination with this product to control angina attacks and have good tolerance. 2. Most patients with β-receptor blockers have good tolerance and efficacy when using this product in combination, but some patients may induce and aggravate hypotension, heart failure and angina. 3. Digitalis This product may increase the blood digoxin concentration, suggesting that the blood concentration of digoxin should be monitored when using it for the first time, adjusting the dose or stopping this product. 4. When drugs with high protein binding rates such as dicoumarins, phenytoin sodium, quinidine, quinine, warfarin, etc. are used with this product, the free concentrations of these drugs often change. 5. When cimetidine is used with this product, the peak plasma concentration of this product increases, so pay attention to adjusting the dose.
