Function and Efficacy

The abbreviation of the active ingredient of this drug is CMX, the molecular formula is C16H17N9O5S3·1/2HCl, and the molecular weight is 529.78. Melting point: It starts to turn yellow at about 130℃, then slowly turns brown, and carbonizes at about 190℃. There is no clear melting point and decomposition point. This drug is white-light orange-yellow crystals or crystalline powder, odorless or slightly odorous. It is easily soluble in formamide, difficult to dissolve in methanol, extremely difficult to dissolve in water, and almost insoluble in ethanol or ether. This drug is a cephalosporin antibiotic with an aminothiazole methoxyiminoacetamide group at the 7th position, namely, aminothiazole cephalosporin hydrochloride injection. Its mechanism of action is to hinder the synthesis of bacterial cell walls, thereby playing a bactericidal role. It has good permeability to the cell wall, is stable to β-lactamase, has a strong affinity for penicillin binding proteins 1A, 1B and 3, and can strongly hinder the cross-linking of cell wall mucopeptides.

Indication

This product is suitable for the following infections caused by ceftriaxone-sensitive Streptococci (except enterococci), Streptococcus pneumoniae, Peptococcus, Peptostreptococcus, Escherichia coli, Citrobacter, Klebsiella, Enterobacter, Serratia, Proteus, Haemophilus influenzae, Bacteroides, etc.: 1. Pneumonia, bronchitis, bronchiectasis infection, secondary infection of chronic respiratory diseases; lung abscess, empyema; 2. Pyelonephritis, cystitis; Bartholinitis, endometritis, adnexitis, pelvic inflammatory disease, parametrial tissue inflammation; 3. Cholangitis, cholecystitis, liver abscess, peritonitis; 4. Secondary infection of burns and surgical trauma; 5. Sepsis; 6. Meningitis.

Usage and Dosage

Adults: Usually 1-2g (titer) of cefmenoxime hydrochloride per day, divided into 2 intravenous injections. For refractory or severe infections, it can be increased to 4g (titer) per day according to symptoms, divided into 2-4 intravenous injections. Children: Usually 40-80mg (titer) of cefmenoxime hydrochloride per day, divided into 3-4 intravenous injections. However, the dosage can be appropriately increased or decreased depending on age and symptoms. For refractory or severe infections, it can be increased to 160mg (titer) per kg per day, divided into 3-4 intravenous injections, and for meningitis, it can be increased to 200mg (titer) per kg per day. For intravenous injection, use [water for injection], [normal saline] or [glucose injection] to dissolve and use. In addition, for adults, a single dose of 0.5g-2g (titer) of this product can be added to the rehydration of sugar solution, electrolyte solution or amino acid preparation, etc., and intravenous drip within 30 minutes to 2 hours. The above dosage can also be considered for children. Add the single dosage to the rehydration solution and perform intravenous drip within 30 minutes to 1 hour. (Precautions for use in dosage) 1. For patients with severe renal dysfunction, the dosage should be appropriately adjusted and the medication interval should be carefully used. (Please refer to [Pharmacokinetics]) 2. When using this preparation, in order to prevent the emergence of drug-resistant bacteria, in principle, the sensitivity should be confirmed and the medication should be used within the shortest period required to treat the disease.

Adverse Reactions

Occasionally it may cause shock, so care should be taken during administration. If symptoms such as discomfort, oral paresthesia, wheezing, dizziness, defecation sensation, tinnitus, sweating, etc. occur, administration should be stopped. If allergic symptoms such as rash, urticaria, erythema, itching, fever, lymphadenopathy, joint pain, etc. occur, administration should be stopped and appropriate measures should be taken. Severe renal impairment, such as acute renal failure, is rare. Agranulocytosis, thrombocytopenia, anemia, eosinophilia, granulocytopenia, and increased S-GOT, S-GPT, AL-P, and LDH rarely occur in very few patients. Jaundice or increased γ-GTP is rare. Gastrointestinal reactions: Pseudomembranous colitis may occur occasionally, manifested as fever, abdominal pain, and mucus, bloody stools, and severe diarrhea as the main symptoms. Endoscopic examination shows pseudomembrane formation, so when abdominal pain or multiple diarrhea occurs, the drug should be stopped immediately and appropriate measures should be taken. Nausea, vomiting, and other symptoms are rare. Patients with renal insufficiency may experience neurological symptoms such as spasms when given in large amounts. Occasionally, stomatitis and thrush may occur. Vitamin K deficiency (hypothrombinemia, bleeding tendency, etc.), vitamin B deficiency (glossitis, stomatitis, loss of appetite, neuritis, etc.). Other reactions: Occasionally, fatigue, staggering, and headache may occur.

Precautions

Patients with a history of anaphylactic shock after using this drug, or those with a history of allergy to the ingredients of this drug or cephalosporin antibiotics.

Drug Interactions

When used in combination with diuretics such as furosemide, renal dysfunction may be aggravated.
