Function and Efficacy

Pharmacological action: 1. Propranolol is a non-selective competitive inhibitor of adrenergic beta receptor blocker. It blocks beta 1 and beta 2 receptors on the heart, antagonizes sympathetic nerve excitation and catecholamine action, reduces the contractile force and contractile speed of the heart, inhibits vascular smooth muscle contraction, reduces myocardial oxygen consumption, and restores the oxygen supply and demand relationship of ischemic myocardium to a low level. It can be used to treat angina pectoris. 2. Inhibits adrenergic excitement of cardiac pacemaker potential and is used to treat arrhythmias. This product can also be used to treat hypertension by blocking central and adrenergic neurons, inhibiting renin release and reducing cardiac output. 3. Competitively antagonizes the effects of isoproterenol and norepinephrine, blocks beta 2 receptors, and reduces plasma renin activity. It can cause bronchospasm. It inhibits insulin secretion, increases blood sugar, masks hypoglycemia symptoms, and delays the recovery of hypoglycemia. 4. It has obvious antiplatelet aggregation effect, which is mainly related to the membrane stabilization effect of the drug and the inhibition of platelet membrane Ca2 transport. Carcinogenicity, mutagenicity and reproductive toxicity: Rats or mice were given 150 mg/kg daily for 18 months, with no obvious toxic reactions and no drug-related carcinogenicity. Reproductive experiments did not show reproductive damage related to the effect of propranolol. When animals were given 10 times the human dose, this product showed embryotoxicity.

Indication

1. As secondary prevention, reduce the mortality rate of myocardial infarction. 2. Hypertension (alone or in combination with other antihypertensive drugs). 3. Exertional angina pectoris. 4. Control supraventricular tachyarrhythmias and ventricular arrhythmias, especially arrhythmias related to catecholamines or caused by digitalis. It can be used to control the ventricular rate of atrial flutter and atrial fibrillation that are not well treated with digitalis, and can also be used for refractory premature contractions to improve patients' symptoms. 5. Reduce the outflow tract pressure difference in hypertrophic cardiomyopathy, relieve symptoms such as angina pectoris, palpitations and syncope. 6. Used in combination with alpha-receptor blockers to control tachycardia in patients with pheochromocytoma. 7. Used to control the rapid heart rate of hyperthyroidism, and can also be used to treat thyroid crisis.

Usage and Dosage

1 Hypertension: Oral, initial dose 10mg, 3-4 times a day, can be used alone or in combination with diuretics. The dose should be gradually increased, the maximum daily dose is 200mg. 2 Angina pectoris: 5-10mg at the beginning, 3-4 times a day; increase 10-20mg every 3 days, can gradually increase to 200mg per day, divided into doses. 3 Arrhythmia: 10-30mg per day, 3-4 times a day. Take before meals and before bedtime. 4 Myocardial infarction: 30-240mg per day, 2-3 times a day. 5 Hypertrophic cardiomyopathy: 10-20mg, 3-4 times a day. Adjust the dose according to needs and tolerance. 6 Pheochromocytoma: 10-20mg, 3-4 times a day. Use for three days before surgery. Generally, receptor blockers should be used first, and propranolol should be added after the drug effect is stable.

Adverse Reactions

The use of this product may cause adverse reactions of the central nervous system such as dizziness, confusion (especially in the elderly), depression, slow reaction, etc.; dizziness (caused by hypotension); slow heart rate (<50 times/minute); less common are bronchospasm and dyspnea, congestive heart failure; even rarer are fever and sore throat (granulocytopenia), rash (allergic reaction), bleeding tendency (thrombocytopenia); when adverse reactions persist, be particularly vigilant against Raynaud's sign such as cold limbs, diarrhea, fatigue, dry eyes, mouth or skin, nausea, numbness of fingers and toes, abnormal fatigue, etc.

Precautions

1 Bronchial asthma. 2 Cardiogenic shock. 3 Heart block (II-III degree atrioventricular block). 4 Severe or acute heart failure. 5 Sinus bradycardia.

Drug Interactions

1 Interaction with antihypertensive drugs: When used in combination with reserpine, this product may cause postural hypotension, bradycardia, dizziness, and syncope. When used in combination with monoamine oxidase inhibitors, it may cause extreme hypotension. 2 When used in combination with digitalis, atrioventricular block may occur and the heart rate may slow down, which requires close observation. 3 When used in combination with calcium antagonists, especially intravenous verapamil, be very vigilant about the inhibition of this product on the myocardium and conduction system. 4 When used in combination with adrenaline, phenylephrine or sympathomimetic amines, it may cause significant hypertension, bradycardia, and atrioventricular block. 5 When used in combination with isoproterenol or xanthine, the latter's efficacy may be weakened. 6 When used in combination with haloperidol, it may cause hypotension and cardiac arrest. 7 When used in combination with aluminum hydroxide gel, the intestinal absorption of propranolol may be reduced. 8 Alcohol may slow down the absorption rate of this product. 9 When used in combination with phenytoin, phenobarbital and rifampicin, the clearance of this product may be accelerated. 10 Combination with chlorpromazine can increase the blood concentration of both drugs. 11 Combination with antipyrine, theophylline and lidocaine can reduce the clearance rate of this product. 12 Combination with thyroxine leads to a decrease in T3 concentration. 13 Combination with cimetidine can reduce the liver metabolism of this product, delay elimination, and increase the blood concentration of propranolol. 14 It can affect blood sugar levels, so when used with hypoglycemic drugs, the latter's dose needs to be adjusted.
