Function and Efficacy

Terbutaline sulfate injection is a beta 2 receptor selective agonist. Its main pharmacological effects are to relax bronchial smooth muscle, inhibit the release of transmitters from mast cells; enhance mucus transport; prevent mucosal edema by inhibiting the release of endogenous transmitters; stimulate mucosal chloride pumps; and increase airway moisture. It has little effect on cardiac excitation, so it does not increase myocardial oxygen consumption, and will not induce or aggravate angina pectoris and arrhythmias.

Appearance

Off-white or light yellow suspension, filled in aluminum bottles with valves.

Indication

Bronchial asthma, chronic asthmatic bronchitis, obstructive emphysema and other lung diseases associated with bronchospasm.

Usage and Dosage

Add 0.25 mg of terbutaline sulfate injection to 100 ml of normal saline and slowly drip intravenously at a rate of 0.0025 mg/min. For adults, take 0.5-0.75 mg per day, divided into 2-3 doses.

Adverse Reactions

Following the recommended dose, the incidence of adverse reactions is low, mostly mild, tolerable, and does not affect continued treatment. Central nervous system: occasional tremor, nervousness, dizziness, headache, occasional drowsiness. Cardiovascular system: occasional palpitations, tachycardia. Respiratory system: chest discomfort. Occasional allergic reactions have been reported. Digestive system: occasional nausea and vomiting. Systemic manifestations: occasional fatigue, facial flushing, sweating, local pain at injection. Elevated liver transaminases and allergic vasculitis are occasionally seen.

Precautions

Patients who are allergic to this product or other adrenergic receptor agonists are contraindicated.

Special Population Medication

Precautions for children: Since there are not enough clinical trials to confirm the safety and effectiveness of the drug in children, terbutaline is not recommended for use in children under 12 years old. Precautions for pregnancy and lactation: In reproductive studies on mice and rabbits, subcutaneous doses of 1500 times the maximum daily dose for adults did not provide any evidence of damage to fertility or effects on the fetus. When terbutaline was given intravenously to baboons near labor, the dose was 4 times the maximum subcutaneous dose for adults, and an increase in maternal and fetal blood sugar was observed, but there are not enough controlled trials to confirm this in pregnant women. Since animal reproductive studies are not always consistent with human responses, pregnant women should consider using it only when they really need it and should carefully weigh the pros and cons. It is not clear whether this product is excreted into human milk, so use it with caution in lactating women. Precautions for the elderly: Since there are not enough clinical trials to confirm the safety and effectiveness of the drug in the elderly, terbutaline is not recommended for use in elderly people over 60 years old.

Drug Interactions

It is recommended that terbutaline sulfate should not be used in combination with other sympathomimetic drugs, as the synergistic effects of the drugs on the cardiovascular system may have adverse effects on patients. Adrenergic receptor agonists should be used with caution in patients who are receiving monoamine oxidase inhibitors and tricyclic antidepressants, as combined use can enhance the effects of adrenergic receptor agonists on the cardiovascular system.
