Function and Efficacy
Through parenteral nutrition, long-chain triglycerides (LCT) and medium-chain triglycerides (MCT) that can be quickly converted can meet the body's energy needs. Among them, long-chain triglycerides (LCT) can also ensure the need for essential fatty acids. Fatty acids are the main energy source for the human body; fatty acid oxidation is an important source of energy in the human body. When the oxygen supply is sufficient, fatty acids can decompose CO2 and H2O in the body and release a large amount of energy, which is used by the body in the form of ATP. Except for brain tissue, most tissues can oxidize fatty acids, especially liver and muscle. Some unsaturated fatty acids cannot be synthesized by the body itself and need to be taken from vegetable oils. They are indispensable nutrients for the body, so they are called essential fatty acids. They are also the precursors of physiologically active substances such as prostaglandins, thromboxanes and leukotrienes. Medium-chain triglycerides (MCT) have a small molecular weight and do not need carnitine to carry them into the mitochondria during metabolism. They are oxidized quickly and thoroughly and can provide energy in the form of coenzyme A and ketone bodies. Medium-chain fatty acids are not easy to be re-esterified and play a complete role. Appearance
This product is a white milky liquid.
Indication
For use in patients who require parenteral nutrition and/or have essential fatty acid deficiency.
Usage and Dosage
Intravenous drip: Unless otherwise specified or based on energy needs, the recommended dose is: 10% of this product is intravenously dripped daily according to body weight. 10-20ml/kg or 20% of this product is 5-10ml/kg, equivalent to 1-2g (2g is the maximum recommended dose) fat/kg. Infusion rate: The maximum rate is 10% of this product per hour. 1.25ml/kg or 20% 0.625ml/kg (equivalent to 0.125g fat/kg). When starting to use this product for parenteral nutrition treatment, it is recommended to use a slower rate, that is, 0.05% fat/kg per hour according to body weight. This product can be infused alone or prepared as an "all-in-one" nutrient mixture for infusion. Other drugs can only be added to this product if compatibility is guaranteed. When infused intravenously. If necessary, this product can be infused together with compound amino acid injection and glucose injection. When this product is infused together with amino acid and/or sugar solution, a separate infusion system and vein should be used. If this product is to pass through a common final infusion channel (bypass, Y-tube), it must be ensured that all solutions are compatible. Filters with a pore size of 0.2 μm cannot be used because fat emulsion particles cannot pass through these filters. Shake well before use! Patients can use this product during parenteral nutrition therapy.
Adverse Reactions
Early adverse reactions that may occur after using this product are: mild increase in body temperature, feeling of fever; feeling of cold, chills, abnormal feeling of heat (flushes) or cyanosis, decreased appetite, nausea, vomiting, difficulty breathing, headache, back pain, bone pain, chest pain, low back pain, abnormal penile erection (rare), increased or decreased blood pressure (hypertension, hypotension), allergic reactions (such as anaphylactic reactions, rash). If these adverse reactions occur, or the serum triglyceride concentration is higher than 3mmol/L during fat emulsion infusion, the infusion should be stopped. If necessary, the dose should be reduced before infusion. If there is a significant reactive increase in blood sugar, the infusion should also be stopped. If there is a serious overdose and carbohydrates are not given at the same time, metabolic acidosis may occur. Pay close attention to the occurrence of overdose syndrome.
Precautions
Severe coagulation disorder, shock and collapse, pregnancy, acute thromboembolism, severe sepsis with acidosis and hypoxia, fat embolism, acute myocardial infarction and stroke, ketoacidosis coma and prediabetic coma. When triglyceride accumulation occurs during infusion, the following are also contraindicated: lipid metabolism disorder, liver dysfunction, renal dysfunction, reticuloendothelial system disorder, acute hemorrhagic necrotizing pancreatitis. General contraindications to parenteral nutrition: acidosis caused by various reasons. Untreated water and electrolyte metabolism disorders (hypotonic dehydration, hypokalemia, water retention), unstable metabolism. Intrahepatic cholestasis.
