Function and Efficacy

This product is a hypoglycemic drug. 1. It stimulates the pancreatic islet beta cells to secrete insulin, which presupposes that the islet beta cells also have a certain ability to synthesize and secrete insulin; 2. It inhibits liver glycogenolysis and gluconeogenesis by increasing portal vein insulin levels or directly acting on the liver, and reduces the generation and output of glucose in the liver; 3. It may also increase the sensitivity of extrapancreatic tissues to insulin and the utilization of sugar (probably mainly through post-receptor effects), so the overall effect is to reduce fasting blood sugar and postprandial blood sugar. In addition, this product also has an antidiuretic effect, which can reduce the clearance of free water. In patients with partial diabetes insipidus, it can enhance the residual antidiuretic hormone effect.

Indication

It is suitable for mild and moderate type Ⅱ diabetes mellitus that is not satisfactory with diet alone, and the patient's pancreatic β cells have a certain insulin secretion function and no serious complications. This product can also be used for central diabetes insipidus.

Usage and Dosage

The usual oral dose is 0.1-0.3g once a day. Start with 0.1-0.2g before breakfast, and increase by 50mg every week. The general dose is 0.3g per day, and the maximum dose is 0.5g per day. Taking it in divided doses can reduce gastrointestinal reactions and improve the control of hyperglycemia. For adults with diabetes insipidus, take 0.1-0.2g per day, once a day, and increase by 50mg every 2-3 days as needed, with a maximum dose of 0.5g.

Adverse Reactions

1. Diarrhea, nausea, vomiting, headache, stomach pain or discomfort may occur; 2. Less common are rashes; 3. Rare but serious symptoms include jaundice, liver damage, bone marrow suppression, granulocytopenia (manifested as sore throat, fever, infection), thrombocytopenia (manifested as bleeding, purpura), etc. 4. It may cause water and sodium retention and hyponatremia.

Precautions

It should not be used in the following situations: 1. Patients with type Ⅰ diabetes; 2. Patients with type Ⅱ diabetes accompanied by stressful situations such as ketoacidosis, coma, severe burns, infection, trauma and major surgery; 3. Patients with liver and kidney dysfunction and heart failure; 4. Those who are allergic to sulfonamides; 5. Patients with leukopenia.

Drug Interactions

1. When taken with alcohol, it can cause abdominal cramps, nausea, vomiting, headache, facial flushing and hypoglycemia. 2. When used with beta-receptor blockers, it can increase the risk of hypoglycemia and mask the symptoms of hypoglycemia, such as increased pulse rate and blood pressure. The possibility of causing this situation is low when using a small amount of selective beta-receptor blockers such as atenolol and metoprolol. 3. Chloramphenicol, guanethidine, insulin, monoamine oxidase inhibitors, phenylbutazone, hydroxyphenylbutazone, probenecid, salicylates, and sulfonamides can enhance the hypoglycemic effect when used with this product. 4. Adrenal cortical hormones, epinephrine, phenytoin sodium, thiazide diuretics, and thyroxine can increase blood sugar levels. When used with this class of drugs, the dosage of this class of drugs may need to be increased. 5. When coumarin anticoagulants are used with this class of drugs, the plasma concentrations of each other increase at first, but then the plasma concentrations of each other decrease, so the dosage of both needs to be adjusted.
