Cyclophosphamide for injection

Function and Efficacy

This product is inactive in vitro. Once it enters the body, it is hydrolyzed by the excessive phosphatase or phosphatase in the liver or tumor, and becomes activated phosphoramide nitrogen mustard. Its mechanism of action is similar to that of nitrogen mustard. It cross-links with DNA, inhibits DNA synthesis, and can also interfere with RNA function. It is a non-specific drug for the cell cycle. This product has a broad anti-tumor spectrum and has inhibitory effects on a variety of tumors.

Ingredients

The main ingredient of this product and its chemical name are: P-[N, N-bis(beta-chloroethyl)]-1-oxo-3-nitrogen-2-phosphorus heterocyclohexane-P-oxide-hydrate.

Appearance

This product is white crystal or crystalline powder.

Indication

This product is a widely used anticancer drug. It has a good effect on malignant lymphoma, acute or chronic lymphocytic leukemia, and multiple myeloma. It also has a certain effect on breast cancer, testicular tumors, ovarian cancer, lung cancer, head and neck squamous cell carcinoma, nasopharyngeal carcinoma, neuroblastoma, rhabdomyosarcoma and osteosarcoma.

Usage and Dosage

Common dosage for adults: single drug intravenous injection, 500-1000mg/m2 per body surface area, add 20-30ml of normal saline, intravenous pulse injection, once a week, 2 times in a row, rest for 1-2 weeks and repeat. Combination medication 500-600mg/m2. Common dosage for children: intravenous injection of 10-15mg/kg each time, add 20ml of normal saline and slowly inject, once a week, 2 times in a row, rest for 1-2 weeks and repeat. It can also be injected intramuscularly.

Adverse Reactions

Bone marrow suppression: Leukopenia is more common than thrombocytopenia, with the lowest value occurring 1 to 2 weeks after medication, and usually recovering after 2 to 3 weeks. It has an impact on liver function. Gastrointestinal reactions: including loss of appetite, nausea and vomiting, which generally disappear after 1 to 3 days of discontinuation of medication. Urinary tract reactions: When large doses of cyclophosphamide are intravenously administered without effective preventive measures, hemorrhagic cystitis may occur, manifested as bladder irritation symptoms, oliguria, hematuria and proteinuria, which is caused by the irritation of the bladder by its metabolite acrolein. However, the incidence is low when cyclophosphamide is used at conventional doses. Other reactions include hair loss, stomatitis, toxic hepatitis, skin pigmentation, menstrual disorders, azoospermia or oligospermia, and pulmonary fibrosis.

Precautions

Anticancer drugs must be used under the guidance of experienced specialists. Patients with bone marrow suppression, infection, liver and kidney damage are prohibited from using or should use with caution. Patients who are allergic to this product are prohibited from using it. Pregnant and lactating women are prohibited from using it.

Special Population Medication

Precautions for children: This product has not been tested and there is no reference literature. Precautions for pregnancy and lactation: It has mutagenic and teratogenic effects, which can cause fetal death or congenital malformations. It is contraindicated for pregnant women. This product can be excreted in breast milk. Breastfeeding must be stopped when starting the medication. Precautions for the elderly: This product has not been tested and there is no reference literature.

Drug Interactions

Cyclophosphamide can reduce pseudocholinesterase in serum and increase serum uric acid levels. Therefore, when used with anti-gout drugs such as allopurinol, colchicine, and probenecid, the dose of anti-gout drugs should be adjusted. In addition, it also enhances the neuromuscular blocking effect of succinylcholine and can prolong apnea. Cyclophosphamide can inhibit cholinesterase activity, thereby prolonging the effect of cocaine and increasing toxicity. Large doses of barbiturates and corticosteroids can affect the metabolism of cyclophosphamide, and their simultaneous use can increase the acute toxicity of cyclophosphamide.
