Function and Efficacy

1 Pharmacology Cephalexin is a first-generation cephalosporin with an antibacterial spectrum similar to that of cephalothin, but its antibacterial activity is poorer than that of the latter. Except for Enterococcus and methicillin-resistant Staphylococci, most strains of Streptococcus pneumoniae, hemolytic Streptococcus, and Staphylococcus aureus that produces or does not produce penicillinase are sensitive to this product. This product has a good antibacterial effect on Neisseria, but Haemophilus influenzae is less sensitive to this product; this product has a certain antibacterial effect on some Escherichia coli, Proteus mirabilis, Salmonella and Shigella. The remaining Enterobacteriaceae, Acinetobacter, Pseudomonas aeruginosa, and Bacteroides fragilis are resistant to this product. Fusobacterium and Veillonella are generally sensitive to this product, and anaerobic Gram-positive cocci are moderately sensitive to this product. 2 Toxicity The oral median lethal dose of cephalexin in mice is 2600 mg/kg.

Appearance

This product is white or off-white.

Indication

It is suitable for acute tonsillitis, pharyngitis, otitis media, sinusitis, bronchitis, pneumonia and other respiratory tract infections, urinary tract infections and skin and soft tissue infections caused by sensitive bacteria. This product is an oral preparation and should not be used for severe infections.

Usage and Dosage

Adult dose: Oral, 250-500 mg once, 4 times a day, the highest dose is 4g per day. Patients with impaired renal function should reduce the dosage according to the degree of renal function impairment. Patients with simple cystitis, skin and soft tissue infections and streptococcal pharyngitis should take 500 mg every 12 hours. Children's dose: Oral, 25-50 mg/kg per day, 4 times a day. Patients with skin and soft tissue infections and streptococcal pharyngitis should take 12.5-50 mg/kg once, twice a day.

Adverse Reactions

Most adverse reactions are short-lived and mild. The most common are gastrointestinal reactions, including diarrhea (16%), increased bowel movement (6%), abdominal pain (3%), nausea (7%), indigestion (3%), and abdominal distension (4%); adverse reactions with an incidence rate of less than 2% include rash, urticaria, drug fever, itching, headache, and dizziness. Laboratory abnormalities include transient increases in ALT, AST, ALP, LDH, bilirubin, BUN, and Cr, transient decreases in platelet and white blood cell counts and eosinophilia, and positive direct Coombs test.

Precautions

Patients with a history of allergy to this product or cephalosporin antibiotics are contraindicated.

Special Population Medication

Precautions for children: Oral administration, 25-50 mg/kg per day, 4 times a day. For patients with skin and soft tissue infections and streptococcal pharyngitis, 12.5-50 mg/kg once, 2 times a day. Precautions for pregnancy and lactation: This product crosses the placenta, so pregnant women should use it with caution; this product can also be excreted through breast milk. Although there has been no report of problems with the use of cephalosporins in lactating women, it should still be used after weighing the pros and cons. Precautions for the elderly: Follow the doctor's advice.

Drug Interactions

1. When used in combination with cholestyramine, the average blood concentration of cephalexin can be reduced. 2. Probenecid can delay the renal excretion of this product, and there are also reports that probenecid can increase the excretion of this product in bile.
