Concentrated Sodium Chloride Injection
Function and Efficacy
Sodium chloride is an electrolyte supplement. Sodium and chloride are important electrolytes in the body, mainly present in extracellular fluid, and play a very important role in maintaining normal blood and extracellular fluid volume and osmotic pressure. The normal serum sodium concentration is 135-145mmol/L, accounting for 92% of plasma cations and 90% of total osmotic pressure, so the amount of plasma sodium plays a decisive role in osmotic pressure. The normal serum chloride concentration is 98-106mmol/L. Sodium and chloride ions in the human body are mainly regulated by the hypothalamus, posterior pituitary and kidneys to maintain the stability of body fluid volume and osmotic pressure.

Appearance
This product is a colorless, clear liquid with a slightly salty taste.
Indication
Water intoxication and severe hyponatremia caused by various reasons. This product can quickly increase the osmotic pressure of the extracellular fluid, thereby moving the water in the intracellular fluid to the outside of the cell. While increasing the volume of the extracellular fluid, it can also increase the osmotic pressure of the intracellular fluid.
Usage and Dosage
In severe hypotonic water loss, the solutes in brain cells decrease to maintain cell volume. If the treatment causes the plasma and extracellular fluid sodium concentration and osmotic concentration to rise rapidly, brain cell damage may occur. It is generally believed that when blood sodium is lower than 120mmol/L, the treatment causes the blood sodium to rise at a rate of 0.5mmol/L per hour, and should not exceed 1.5mmol/L per hour. When blood sodium is lower than 120mmol/L or central nervous system symptoms occur, 3% to 5% sodium chloride injection can be given by slow drip. It is generally required to increase the blood sodium concentration to above 120mmol/L within 6 hours. Sodium supplementation (mmol) = [142-actual blood sodium concentration (mmol/L)] x body weight (kg) x 0.2. When blood sodium rises to above 120-125mmol/L, isotonic solution can be used instead, or hypertonic glucose injection or 10% sodium chloride injection can be added to the isotonic solution as appropriate.
Adverse Reactions
1. Excessive and rapid infusion can cause water and sodium retention, resulting in edema, increased blood pressure, increased heart rate, chest tightness, dyspnea, and even acute left heart failure. 2. Excessive and rapid administration of hypotonic sodium chloride can cause hemolysis, cerebral edema, etc.
Precautions
It is contraindicated in the following situations: ① Edema diseases, such as nephrotic syndrome, ascites due to liver cirrhosis, congestive heart failure, acute left heart failure, cerebral edema and idiopathic edema; ② Oliguria in acute renal failure and poor response to diuretics in chronic renal failure; ③ Hypertension and hypokalemia; ④ Hyperosmotic or isotonic water loss.
Special Population Medication
Precautions for children: The amount and speed of fluid replacement should be strictly controlled. Precautions for pregnancy and lactation: Contraindicated for pregnancy-induced hypertension syndrome. Precautions for the elderly: The amount and speed of fluid replacement should be strictly controlled.
