Aspirin Enteric-coated Capsules
Function and Efficacy
Analgesic effect: It is mainly through inhibiting the synthesis of prostaglandins and other substances that can make pain sensitive to mechanical or chemical stimulation (such as bradykinin, histamine), and is a peripheral analgesic. However, the possibility of central analgesia (possibly acting on the hypothalamus) cannot be ruled out. Anti-inflammatory effect: The exact mechanism is still unclear. It may be due to the fact that this product acts on inflamed tissues and inhibits the synthesis of prostaglandins or other substances that can cause inflammatory reactions (such as histamine) to play an anti-inflammatory role. Inhibition of the release of lysosomal enzymes and leukocyte chemotaxis may also be related to it. Antipyretic effect: It may act on the hypothalamic thermoregulatory center to cause peripheral vasodilation, increased skin blood flow, sweating, and increased heat dissipation to play an antipyretic role. This central effect may be related to the inhibition of prostaglandin synthesis in the hypothalamus. Anti-rheumatic effect: The anti-rheumatic mechanism of this product, in addition to antipyretic and analgesic effects, is mainly anti-inflammatory. Inhibition of platelet aggregation: It works by inhibiting platelet cyclooxygenase and reducing the production of prostaglandins.

Appearance
This product is a hard capsule containing white granules.
Indication
(1) Analgesic and antipyretic: It can relieve mild or moderate pain, such as headache, toothache, neuralgia, muscle pain and menstrual pain. It is also used to reduce fever for colds and flu. This product can only relieve symptoms and cannot treat the cause of pain and fever, so other drugs must be used at the same time to treat the cause. (2) Anti-inflammatory and anti-rheumatic: It is a commonly used drug for the treatment of rheumatic fever. It can reduce fever, improve joint symptoms and reduce erythrocyte sedimentation rate, but it cannot eliminate the basic pathological changes of rheumatic fever, nor can it treat and prevent heart damage and other complications. (3) Arthritis: In addition to rheumatoid arthritis, this product is also used to treat rheumatoid arthritis and can improve symptoms, but the cause must be treated at the same time. In addition, this product is also used for osteoarthritis, ankylosing spondylitis, juvenile arthritis and other non-rheumatic inflammatory diseases.
Usage and Dosage
Oral administration to inhibit platelet aggregation: 0.1g (1 tablet) at a time, once a day, or as directed by a doctor.
Adverse Reactions
1 Common adverse reactions: gastrointestinal reactions such as nausea, vomiting, upper abdominal discomfort or pain. 2 Rare adverse reactions: gastrointestinal bleeding or ulcers, bronchospasmodic hypersensitivity reactions, skin reactions, liver and kidney damage.
Precautions
1. Patients with active gastrointestinal ulcer disease, gastrointestinal bleeding and other active bleeding are contraindicated; 2. Patients with hemophilia or thrombocytopenia are contraindicated; 3. Patients who are allergic to this product or other non-steroidal anti-inflammatory drugs or have a history of allergy, especially those with asthma, neuroangioedema or shock are contraindicated.
Special Population Medication
Precautions for children: Use with caution in children, especially those with fever and dehydration, as toxic reactions are likely to occur. Acute febrile diseases, especially influenza and chickenpox, may be associated with Reye’s Syndrome when using aspirin enteric-coated microparticle capsules. Severe cases can be fatal, which is rare in China. Precautions for pregnancy and lactation: Not clear. Precautions for the elderly: When this product is used for antipyretic, analgesic, and anti-rheumatic purposes, elderly patients are prone to toxic reactions when taking large doses due to decreased renal function, so it should be used with caution.
Drug Interactions
1. The efficacy is not enhanced when used with other non-steroidal anti-inflammatory analgesics, because this product can reduce the bioavailability of other non-steroidal anti-inflammatory drugs. In addition, gastrointestinal side effects (including ulcers and bleeding) are increased; in addition, due to the enhanced inhibitory effect on platelet aggregation, the risk of bleeding in other parts can also be increased. Long-term and large-scale use of this product with acetaminophen may cause kidney diseases including renal papillary necrosis, renal cancer or bladder cancer. 2. When used with any drug that can cause hypoprothrombinemia, thrombocytopenia, reduced platelet aggregation function or gastrointestinal ulcer bleeding, there may be a risk of aggravating coagulation disorders and causing bleeding. 3. When used with anticoagulants (dicoumarol, heparin, etc.) and thrombolytics (streptokinase, urokinase), the risk of bleeding may be increased. 4. Urine alkalinizing drugs (sodium bicarbonate, etc.) and antacids (long-term and large-scale use) can increase the excretion of this product in urine, causing a decrease in blood drug concentration. However, when the blood concentration of this product has reached a stable state and the alkaline drug is discontinued, the blood concentration of this product can be increased to a toxic level. Carbonic anhydrase inhibitors can alkalinize urine, but can cause metabolic acidosis, which can not only reduce the blood concentration, but also increase the amount of this product that penetrates into the brain tissue, thereby increasing toxic reactions. 5 Uric acid drugs can reduce the excretion of this product and increase its blood concentration. Patients whose blood concentration of this product has reached a stable state may increase the blood concentration of this product and increase toxic reactions after adding uricidifying drugs. 6 Glucocorticoids (hormones for short) can increase the excretion of salicylates. In order to maintain the blood concentration of this product when used together, the dosage of this product should be increased if necessary. Long-term use of this product and hormones, especially when used in large quantities, increases the risk of gastrointestinal ulcers and bleeding. For this reason, it is not recommended to use these two drugs at the same time in clinical practice. 7 The hypoglycemic effect of insulin or oral hypoglycemic drugs can be enhanced and accelerated by using them together with this product. 8 When used with methotrexate (MTX), it can reduce the binding of methotrexate to proteins, reduce its excretion from the kidneys, increase blood drug concentrations and increase toxic reactions. 9 The uric acid excretion effect of probenecid or sulfinpyrazone can be reduced by the simultaneous use of this product; when the blood concentration of salicylate is 50mu;g/ml, it is significantly reduced, and even more so when it is 100-150mu;g/ml. In addition, probenecid can reduce the clearance rate of salicylate from the kidneys, thereby increasing the latter's blood concentration.
