Cefuroxime Capsules (Fanjiefu)

This product is a first-generation cephalosporin, which has good antibacterial effect on some strains of Gram-positive cocci such as non-penicillinase-producing and penicillinase-producing Staphylococcus aureus, coagulase-negative Staphylococcus, group A hemolytic Streptococcus, Streptococcus pneumoniae and viridans Streptococcus. Anaerobic Gram-positive bacteria are mostly sensitive to this product, and Bacteroides fragilis is resistant to this product. Methicillin-resistant Staphylococci and Enterococci are resistant to this product. The effect of this product on Gram-positive and Gram-negative bacteria is similar to that of cephalexin. This product has a certain effect on Neisseria gonorrhoeae, and is also active against enzyme-producing Neisseria gonorrhoeae; its activity against Haemophilus influenzae is poor.

The main ingredient of this product is cefradine. Its chemical name is (6R,7R)-7[(R)-2-amino-2-(1,4-cyclohexenyl)acetylamino]-3-methyl-8-oxo-5-thia-1-azabicyclo[4,2,0]oct-2-ene-2-carboxylic acid.

The content of this product is white to off-white or light yellow powder or granules.

It is suitable for acute pharyngitis, tonsillitis, otitis media, bronchitis, pneumonia and other respiratory tract infections, urogenital tract infections and skin and soft tissue infections caused by sensitive bacteria. This product is an oral preparation and should not be used for severe infections.

Common dosage: Oral. Adults take 0.25-0.5 mg once every 6 hours. For severe infections, the dosage can be increased to 1 g, but the total daily dosage should not exceed 4 g. It is best to take it after meals. Children take 6.25-12.5 mg/kg per day per kilogram of body weight, divided into equal doses every 6 or 12 hours. In severe cases, the dosage can be increased to 0.75-1 mg/kg per day per kilogram of body weight. Even if the condition begins to improve, the patient still needs to complete a course of treatment; for patients who need long-term treatment of prostatitis, the dosage should not be lower than the above common dosage.

The adverse reactions of this product are mild and the incidence is low, about 6%. Gastrointestinal reactions such as nausea, vomiting, diarrhea, and upper abdominal discomfort are more common. The incidence of drug rash is about 1% to 3%. Pseudomembranous colitis, eosinophilia, positive reaction to direct Coombs test, peripheral blood leukocytopenia and neutrophil granulocyte reduction are seen in individual patients. A small number of patients may experience temporary increase in blood urea nitrogen, serum aminotransferase, and serum alkaline phosphatase.

This product is contraindicated in patients who are allergic to cephalosporins or have a history of anaphylactic shock or immediate reaction to penicillin.

Precautions for children: It may cause hematuria. Children are susceptible to the disease. Children should be cautious when using this product and should be monitored. Precautions for pregnancy and lactation: This product can enter the fetal blood circulation through the placental barrier and enter the breast milk. Pregnant and lactating women should use it with caution. Precautions for the elderly: Elderly patients with impaired renal function should adjust the dosage or the interval between doses appropriately.

1. Cephalosporins can delay the excretion of phenytoin sodium in the renal tubules. 2. Phenylbutazone combined with cephalosporin antibiotics can increase renal toxicity. 3. Combined with strong diuretics can increase renal toxicity. 4. Combined with mecillinam, it has a synergistic effect on gram-negative rods such as Escherichia coli and Salmonella. 5. Probenecid can delay the excretion of this product.
