Function and Efficacy

This product is a penicillin antibiotic. The antibacterial spectrum is the same as penicillin. It has antibacterial activity against most Gram-positive bacteria, Gram-negative cocci, individual Gram-negative bacilli (such as Haemophilus), spirochetes and actinomycetes, but most Staphylococcus strains (90%) including Staphylococcus aureus and coagulase-negative Staphylococci can produce beta-lactamase to hydrolyze and inactivate this product. The activity of this product against most sensitive strains is 2 to 5 times weaker than that of penicillin. It has no antibacterial effect on strains that produce penicillinase. The mechanism of action of this product is to inhibit the synthesis of bacterial cell walls, causing the bacteria to rupture and dissolve rapidly.

Appearance

Penicillin V potassium tablets are film-coated tablets that appear white after removing the coating.

Indication

This product is suitable for mild and moderate infections caused by penicillin-sensitive strains, including tonsillitis, pharyngitis, scarlet fever, erysipelas, etc. caused by streptococci; bronchitis, pneumonia, otitis media, sinusitis caused by pneumococci, and skin and soft tissue infections caused by sensitive staphylococci, etc. This product can also be used for spirochetal infections and as a preventive drug for recurrent rheumatic fever and infective endocarditis.

Usage and Dosage

Oral. Streptococcal infection in adults: 125-250 mg once, once every 6-8 hours, for a course of 10 days. Pneumococcal infection: 250-500 mg once, once every 6 hours, for a course of at least 2 days after the fever subsides. Staphylococcal infection, spirochete infection (Fensen pharyngitis): 250-500 mg once, once every 6-8 hours. Prevention of recurrence of rheumatic fever: 250 mg once, twice a day. Prevention of endocarditis: Take 2g of this product orally 1 hour before tooth extraction or upper respiratory tract surgery, and take another 1g after 6 hours (the dose for children under 27kg is halved). For children, take 2.5-9.3mg/kg once, once every 4 hours, or 3.75-14mg/kg once, once every 6 hours, or 5-18.7mg/kg once, according to body weight.

Adverse Reactions

1. Common gastrointestinal reactions such as nausea, vomiting, upper abdominal discomfort, diarrhea, and black hairy tongue. 2. Allergic reactions: rash (especially prone to occur in patients with infectious mononucleosis), urticaria and other serum sickness-like reactions, laryngeal edema, drug fever, and eosinophilia. 3. Superinfection: Long-term or large-scale use of this product can cause penicillin-resistant Staphylococcus aureus, Gram-negative rods, or Candida albicans infection (brown or even black tongue coating). 4. Rare hemolytic anemia, transient increase in serum aminotransferase, leukopenia, thrombocytopenia, neurotoxicity, and nephrotoxicity, etc.

Precautions

1. It is contraindicated for patients with positive penicillin skin test reaction, those who are allergic to this product and other penicillins, and patients with infectious mononucleosis; 2. It is used with caution by pregnant women;

Drug Interactions

1. Probenecid, aspirin, indomethacin, phenylbutazone, and sulfonamides can reduce the excretion of this product in the renal tubules, thereby increasing the blood concentration of this product, prolonging the blood elimination half-life (t1/2?), and possibly increasing the toxicity. 2. When this product is used in combination with allopurinol, the incidence of rash increases significantly, so it should be avoided. 3. This product should not be used in combination with acetaldehyde dehydrogenase inhibitors such as disulfiram. 4. When this product is used in combination with chloramphenicol for bacterial meningitis, the incidence of long-term sequelae is higher than when either is used alone. 5. This product can stimulate estrogen metabolism or reduce its enterohepatic circulation, thereby reducing the effect of oral contraceptives. 6. Antibiotics such as chloramphenicol, erythromycin, tetracyclines, and antibacterial drugs such as sulfonamides can interfere with the bactericidal activity of this product, so it is not suitable to be used in combination with this product, especially in the treatment of meningitis or severe infections that urgently need bactericidal drugs. 7. This product can enhance the effect of warfarin. Clavulanic acid can enhance the antibacterial activity of this product against B-lactamase-producing bacteria; aminoglycoside antibiotics can generally enhance the in vitro bactericidal effect of this product against Enterococcus faecalis at sub-inhibitory concentrations.
